Intake and Assessment. Client intake will occur preferably at the first contact, and will involve information gathering and completion/dissemination of all basic paperwork to include, but not limited to, all required eligibility documentation, insurance information, demographic information, Consent for the Release of Medical Information, Client Rights and Responsibilities, and Grievance Procedures and forms. In addition, the Referral Specialist shall determine where the client was originally diagnosed and the date and location of the client’s last medical appointment. If the client has never been treated for HIV/AIDS or has not received such care for a period of more than six (6) months, all necessary additional information shall be obtained to further assess the existence of any barriers to care.

Intake and Assessment. Intake refers to the initial process that occurs when a participant enters the program seeking employment, beginning with basic data collection that includes: demographic information, eligibility determination, and program orientation. Additionally, prior to enrollment in a selected training and/or course of study, participants will receive a comprehensive assessment and needs analysis to determine their individual strengths, interests, aptitudes, and abilities, as well as risks and barriers. Career Development Plan: Program staff will work with participants to develop an Individualized Career Development Plan (ICP) with appropriate intervention strategies towards goal attainment. The ICP will be re-evaluated on an ongoing basis. The ICP should be mutually signed by Provider staff and participant, developed using assessments, job market outlook and coordination of services, and identify both short-term and long-term goals. NOTE: Documentation of individual assessments performed and ICP Plans created must be maintained in the case file. Invoicing Documentation: Participant signed attestation for completion of assessment and review of results.
