Extended PTO Requests. Each year beginning January 2nd through February 28th, Inspira shall accept requests for Extended PTO. All requests for Extended PTO shall be submitted by the employee on a PTO request form. Space for a 1st, 2nd and 3rd choice shall be provided. The 2nd and 3rd choice will only be considered if the 1st choice cannot be granted. If two or more employees request the same days off then the employee with the greatest bargaining unit seniority will be given their first choice. No more than one (1) episode of a two consecutive week PTO will be granted to each employee during the time of May 1st to September 15th. Extended PTO shall not be granted for the time period between December 15th and January 3rd, unless a written request is reviewed by the Director of Nursing, and approved by the VP of Patient Care Services. Inspira shall approve/deny/address all requests for Extended PTO submitted by February 28th no later than March 16th. Such requests shall not be unreasonably denied. A request consisting of greater than two consecutive weeks must be approved by the Nursing Director. Requests for Extended PTO during the week of a holiday will be considered only after individual holiday commitments have been addressed and will be granted solely on patient needs. Requests for Extended PTO may be made after February 28th and will be considered on a first-come, first served basis. Such approval lies within the sole discretion of Inspira based upon patient needs, standards of care, and fair staffing practices.

Extended PTO Requests. To the extent business requirements permit, written employee requests to the employee's immediate supervisor for specific periods in which to take paid time off of three (3) or more consecutive shifts (“Extended PTO”) will be submitted not less than thirty (30) days nor more than one hundred eighty (180) days prior to the requested dates of Extended PTO and will be honored on a first come, first serve basis; provided, however, when more than one employee submits an Extended PTO request in the same week for the same period, seniority shall prevail.

  • Leave Requests All requests for leaves of absence must be in writing.

  • Service Requests In support of services outlined in this Agreement, the Service Provider will respond to service related incidents and/or requests submitted by the Customer within the following time frames:  0-8 hours (during business hours) for issues classified as High priority.  Within 48 hours for issues classified as Medium priority.  Within 5 working days for issues classified as Low priority. Remote assistance will be provided in-line with the above timescales dependent on the priority of the support request.

  • Special Requests Additional Services will be considered by request and billed separately: -Waxing -Shampooing of rugs and carpets -Additional Cleaning of areas or additional cleaning services not quoted in the Master Agreement’s Schedule, Building Areas and Janitorial Standards, and/or “Scope of Work” per agency.

  • Offer and Acceptance of Weekend Overtime (a) The Employer is committed to providing reasonable notice to Employees of an offer / cancellation of weekend overtime. To this end, notice will generally be provided prior to the normal meal break on Thursday. Where the Employer is unable to give such notice the Employer may offer I cancel such overtime by notifying affected Employees before the finish time of ordinary hours on Friday.

  • Access Requests (a) Each party agrees to reasonably cooperate with the other in connection with requests to access or amend Personal Data.

  • Bona Fide Request/New Business Request Process for Further Unbundling 6.1 BellSouth shall, upon request of <<customer_name>>, provide to <<customer_name>> access to its network elements at any technically feasible point for the provision of <<customer_name>>'s telecommunications service where such access is necessary and failure to provide access would impair the ability of <<customer_name>> to provide services that it seeks to offer. Any request by <<customer_name>> for access to a network element, interconnection option, or for the provisioning of any service or product that is not already available shall be treated as a Bona Fide Request/New Business Request (BFR/NBR), and shall be submitted to BellSouth pursuant to the BFR/NBR process.

  • Additional Request Requirements In addition to the requirements of Subparagraph 22A, the request for a WA must provide:

  • Requesting Price Increase/Required Documentation Contractor must submit a written notification at least thirty (30) calendar days prior to the requested effective date of the change, setting the amount of the increase, along with an itemized list of any increased prices, showing the Contractor’s current price, revised price, the actual dollar difference and the percentage of the price increase by line item. Price change requests must include H-GAC Forms D Offered Item Pricing and E Options Pricing, or the documentation used to submit pricing in the original Response and be supported with substantive documentation (e.g. manufacturer's price increase notices, copies of invoices from suppliers, etc.) clearly showing that Contractor's actual costs have increased per the applicable line item bid. The Producer Price Index (PPI) may be used as partial justification, subject to approval by H-GAC, but no price increase based solely on an increase in the PPI will be allowed. This documentation should be submitted in Excel format to facilitate analysis and updating of the website. The letter and documentation must be sent to the Bids and Specifications manager, Xxxxxxx Xxxxxx, at Xxxxxxx.Xxxxxx@x-xxx.xxx Review/Approval of Requests If H-GAC approves the price increase, Contractor will be notified in writing; no price increase will be effective until Contractor receives this notice. If H-GAC does not approve Contractor’s price increase, Contractor may terminate its performance upon sixty (60) days advance written notice to H-GAC, however Contractor must fulfill any outstanding Purchase Orders. Termination of performance is Contractor’s only remedy if H-GAC does not approve the price increase. H-GAC reserves the right to accept or reject any price change request.

  • EXTENSION OF USE COMMITMENT The Contractor agrees to honor all orders from State Agencies, political subdivisions and others authorized by law (see Section 25 Extension of Use) which are in compliance with the pricing, terms, and conditions set forth in the Contract. Any unilateral limitations/restrictions imposed by the Contractor on eligible Authorized Users will be grounds for cancellation of the Contract.

  • Employee-Requested Schedule Changes Overtime-eligible employees’ workweeks and work schedules may be changed at the employee’s request and with the Employer’s approval, provided the Employer’s business and customer service needs are met and no overtime expense is incurred.
