Dental/Optical Insurance. 15.1 The Township shall provide and pay for a dental plan for the employee and dependents. The plan will be the Delta Dental Preferred Option Plus Plan. This plan includes an orthodontic rider which will cover dependents up to age nineteen (19) years of age with a maximum lifetime benefit of $1,500. There shall be no deductible and the plan will have a twelve hundred ($1,200.00) dollar limit on benefits per person per contract year. Dental/optical coverage is available to all eligible Federation members who do not receive district health insurance on an employee-paid basis, under the same terms and conditions set forth in Article X, Section 10.01.

Dental/Optical Insurance. 8.2.1 Effective July 1, 1997, the District shall reimburse teachers covered by this Agreement for dental/optical expense incurred by the teacher or the teacher's dependent (as dependent is defined by the Internal Revenue Service). The maximum amount of reimbursement per fiscal year (July 1 through June 30) shall be Four Hundred Dollars ($400.00) for 2001-02, Four Hundred and Fifty Dollars (450.00) for 2002-03, and Five Hundred Dollars ($500.00) for 2003-04. To be eligible for reimbursement, expenses must have been actually paid by the teacher and not reimbursed or reimbursable from any other source. Reimbursement will be made upon submission of a properly completed claim form. At the end of any fiscal year (June 30) any unused balance, up to a maximum of Seventy-five Dollars ($75.00), in a teachers' dental/optical account shall be carried forward to the next fiscal year.
Dental/Optical Insurance. Each full-time employee will be provided with dental/optical insurance, as provided in Appendix "A". Regular part-time employees who are budgeted to work 20 hours or more per week may participate in the “in house” dental/optical insurance plan on a pro-rated basis.
