OPTICAL. The Employer agrees to provide Optical coverage for active full- time employees in the amount of one hundred twenty-five dollars ($125.00) every twenty-four (24) month period, for the cost of frames, lenses and the fitting of prescription glasses when recommended by a doctor or optometrist. Sunglasses or glasses for cosmetic purposes are not included nor is the cost of eye examinations. While this Appendix is intended to provide an overview of all benefit coverages the insurer’s plan documents will ultimately govern the administration of these benefits. RRSP‌ The Employer agrees to contribute the following amounts to a Registered Retirement Savings Plan for any full-time employee who agrees to contribute an equal amount as follows: Employer Employee Year 1 $550.00 $550.00 Year 2 $550.00 $550.00 Reflect yearly Employer and Employee contribution match. Such amounts shall be divided by the amount of pay periods for each year and shall be adjusted accordingly.
OPTICAL. Effective January 1, 2019, eligible employees will be reimbursed up to eighty (80%) percent of the cost of frames, lenses, and the fitting or prescription eye glasses when recommended by a doctor or optometrist up to a maximum of two hundred ($200) dollars in each two (2) year period of active employment. The cost of prescription contact lenses will be covered in the same amount. Eligible employees must have five (5) years of continuous service and must have worked a minimum of six hundred (600) hours in each of the two calendar years preceding any reimbursement. This plan provides employee coverage only. DRUG PRESCRIPTION Effective, January 1, 2019, eligible employees will be reimbursed one hundred percent of prescription drugs to a yearly maximum of $1000 (one thousand) dollars. Eligible employees must have five (5) years of continuous service and must have worked a minimum of six hundred (600) hours in each of the two (2) calendar years preceding any reimbursement. This plan provides single coverage only.
OPTICAL. CARRIER LEVEL 1 (OC-1): The optical signal that results from an optical conversion of an electrical STS-1 signal (51.840 Mb/s). This signal forms the basis of the interface. OC-3: Optical Carrier level 3 signal operating at 155.520 Mb/s. OC-12: Optical Carrier level 12 signal transmitting at 622.080 Mb/s. OC-48: Optical Carrier level 48 signal transmitting at 2488.32 Mb/s. POINT OF PRESENCE (POP): A physical location where a long distance carrier terminates lines before connecting to the local exchange carrier, another carrier, or directly to a customer.
OPTICAL. For those part-time employees who have one (1) or more years of service and worked six hundred (600) hours in the previous calendar year, the Owner/Company will provide optical coverage of one hundred fifty ($150) dollars every two (2) years for the employee and his/her dependents up to age 18.
OPTICAL. Active full time employees and their eligible dependents shall be eligible for a maximum of $175 every two (2) years. Payment will be made upon the receipt of proof of purchase. APPENDIX “C” PHARMACY OPERATIONS
OPTICAL. The City will provide the cost of the Full-Family plan as set forth in the policy for a comprehensive optical insurance plan allowing for full vision care subject to a Ten Dollar ($10.00) deductible at all participating optometrists.
OPTICAL. Effective July 9, 1997, full time employees and their eligible dependents shall be eligible for a maximum of $150 every two (2) years. Effective May 1, 2000 this shall increase to a maximum of $175 every two (2) years. Payment will be made upon the receipt of proof of purchase APPENDIX “C” Letter of Understanding Between: No Frills Franchise Owners (“The Owners”) And United Food and Commercial Workers Canada, Local 1000A (“The Union”) The purpose of this Letter of Understanding is to clarify the application of the scheduling and availability of part time employees under Article 2.03 of the collective agreement. The parties continue to recognize that the following four principles apply to the scheduling and availability of part time employees and believe this Letter of Understanding is consistent with those principles: • Employees do not have the right to exercise a preference of hours within their weekly schedule of work. • Employees may be scheduled up to 24 hours in a week but an Owner is not obliged to do so. • Owners have no obligation to create shifts to suit the availability of an employee. • Owners determine the length of shifts.
OPTICAL. The Board shall provide optical insurance for employees in Classification #110, #120, and #130 who meet the eligibility requirements of the insurance carrier. The plan will be as provided by Michigan Employee Benefit Services (MEBS) or comparable coverage.
OPTICAL. The City agrees to pay the premiums for full family optical coverage for employees covered by this Agreement for the life of this Agreement.
OPTICAL. The District shall pay the full premium for optical benefits for each administrator and their eligible dependents. The carrier will be selected by the District.
