Community Learning Disability Team. Under the NHS and Community Care Xxx 0000 local authorities have the lead role for assessing and commissioning services to meet the social care needs of people with learning disabilities. Health Services work with social services, housing and other agencies in supporting adults with learning disabilities and their families to maximise their potential and lead valued lives.
Community Learning Disability Team. The Community Learning Disability Team is composed of care managers, administrative staff and access workers employed by the Council. Specialist community learning disability nurses, occupational therapists and their assistants, speech and language therapists and their assistants and physiotherapists in the team, are employed by the PCT. The Team Manager and the Council’s Health and Disabilities Services Manager will have Honorary Contracts with the PCT. Learning Disability staff employed by the PCT will be seconded to the Council. These staff will retain NHS terms and conditions and will be managed by the Team Manager. The Partners will need to ensure that the following are established: ▪ The role and function of each group of professionals on the team ▪ The agreed establishment of each professional group in the team ▪ Satisfactory clinical supervision for health professionals on the team ▪ Satisfactory clinical governance arrangements for the team as a whole ▪ Satisfactory cover arrangements for therapists during periods of annual leave, study leave or other time away from the team.
