Patients without Insurance. Payment for dental services are due at the time of treatment.
Patients without Insurance. Whenever possible, we provide an estimate of fees for needed treatment and payment is expected at each visit for services rendered. All Patients: Please note, for procedures requiring more than one hour of chair time, more than one appointment, we ask that payment be paid in full at the start of treatment or half of the expected copayment be paid at the first scheduled appointment with the remainder of the copayment due at the next scheduled appointment unless other arrangements have been made.
Patients without Insurance. 1. We request that 100% of the first visit be paid at the time of the first visit.
Patients without Insurance. If you do not have insurance you may pay at the time of service, or you may use one of our extended payment plans, once your application has been approved. Payment for visits must be received in full at the time of service to allow us time to process an application, if necessary. PAYMENT OPTIONS -CASH, CHECK -MASTERCARD, VISA, DISCOVER, or AMERICAN EXPRESS -EXTENDED PAYMENT PLANS We are pleased to offer extended payment plans and 12-month interest free financing. This allows our patients to make monthly payments, instead of having to pay in full at the time of service. If you would like to make extended payments for services provided at our office, simply fill out an application form to establish your line of credit. There is NO CHARGE for completing the application and there is NO ANNUAL FEE. We encourage everyone to establish charge privileges. If at any time in the future you, or other family members, wish to take advantage of the charge plans your credit is already established.
Patients without Insurance. All patients without insurance are required to pay in full for the service rendered at the time of the appointment.
Patients without Insurance. Payment in full is expected at the time of service. We accept cash, check, Visa, MasterCard, and Care Credit. There will be a $35.00 charge for all returned checks. We offer a 5% discount for payments in full made in cash or by check on the day of service. We also offer a 5% senior discount. OVER PAST DUE ACCOUNTS / COLLECTION AGENCY: If you have a balance on your account that is more than 90 days old when you arrive for an appointment, you will need to pay that balance as well as any co- pays and deductibles that are due on the current visit. Please be aware that Xxxxxxx Family Dental utilizes a collection agency for unpaid bills. If your account is transferred to collections, any and all fees assessed by the agency will be added to the balance on your account. If your account is turned over to collections, you will be dismissed from the practice and will have to contract with a new dentist. MISSED APPOINTMENTS / LATE CANCELLATIONS: Office visit cancellations require 48 hours notice prior to the appointment, or you may be charged a $50 cancellation fee. Excessive abuse of scheduled appointments may result in discharge from the practice.
Patients without Insurance. Our fees cannot always be determined in advance, since they depend on the services rendered. You will need to pay the charges at the time of services.
Patients without Insurance. Financing options are available and facilitated by our financial coordinator. If you choose to forego these options, charges are required to be paid for in full at the time of surgery. An estimate will be given to you at your examination/consultation appointment or when the appointment is scheduled. MEDICARE: Our doctors are not Medicare providers therefore our office is unable to bill Medicare for any services. Medicare eligible patients will be given a Medicare Private Contract with information regarding our Medicare agreement.
