Patient Insurance. The trial will be covered by a participant insurance in case the trial site (clinic) does not cover the study by its liability insurance (Haftpflichtversicherung). All subjects (parents) will be informed about their rights and obligations in regard to insurance policies before participating in the study. A copy of the insurance policies will be handed out to each patient (parents).

Patient Insurance. For all patients included in the study an insurance cover will be affected in accordance with the legal regulations. An insurance certificate and conditions will be provided to the Investigator prior to start of patient recruitment. Before the beginning of the clinical trial, the Sponsor is obligated to arrange the insurance on the basis of which any harm will be paid that would result from the conduct of the clinical trial. The insurance certificate is an appendix of this agreement as its integral part. Insurance coverage refers to all patients participating in the study and to any health damages caused by the applied medication or by procedures performed on the body of the insured person in connection with the clinical study. The Investigator will inform all patients about their obligations arising from the general insurance conditions: 1. During the course of the clinical study, the patient must inform the Investigator about any other medical treatment he/she undergoes. 8. Patient Insurance. If applicable, patient insurance will be secured according to national or local requirements. Where applicable, the name and address of the relevant insurance company, the certificate of insurance, the policy number and the sum insured will be provided in the Investigator’s File.
Patient Insurance. The Parties acknowledge and agree that the compensation payable under Section 3.1 and Exhibit B is an administrative charge payable to Vendor and shall not cover the actual vaccine administration to patients.   • Accident Insurance It is highly recommended that either the Sending Institution or the Receiving Organisation/Enterprise provide insurance coverage to the trainee, and fill in the information in Table B or C accordingly.   • Health and Accident Insurance Unit members shall continue to be covered under the State's Group Health and Accident Insurance plan currently in effect pursuant to the provisions of Chapter 32A of the General Laws as amended or as such plan may be made available under applicable law of the Commonwealth.   • Travel Accident Insurance We agree to provide you with Travel Accident Insurance at no direct cost to you. You, your spouse and unmarried dependent children will be automatically insured against accidental bodily injuries or death while riding in any aircraft or land or water conveyance operated by a common carrier licensed to carry passengers for hire provided the full travel fare(s) has been charged to your Account. Death benefits will be paid to the estate of the insured; all other benefits will be paid to the insured. This insurance is subject to cancellation without prior notice. You understand and agree that the Certificate of Insurance controls all insurance terms and conditions to the exclusion of any statements made in this Agreement regarding limitations, exclusions, and claims procedures.

  • Trauma Insurance All employees will be covered by an Incolink administered lump sum insurance policy providing financial compensation in the event of a major work related (ie. WorkCover) accident resulting in death or permanent total disablement. The full and precise conditions of this cover will be in accordance with the terms of the policy, but in general will provide that, in the event of a workplace accident occurring which results in either the death or total permanent disablement of a worker covered by this Agreement, a lump sum payment as specified below will made. The defined payments are: With dependants $250,000 Without dependants $150,000 This benefit has been agreed to by the company on the grounds that premium costs have been set at $7 per week/worker and will not exceed that amount. In the event of insurance costs rising, it is agreed that the table of defined benefits will be reduced so as to maintain the $7 premium figure. To maintain this cover the company agrees to pay the amounts every week for each employee.

  • Retirement Insurance A teacher retiring from the District and under the provisions of Teachers’ Retirement Association (TRA) is eligible to continue to participate in group insurance programs (health insurance, dental insurance, life insurance, supplemental life insurance) as permitted under the insurance policy provisions provided the teacher pay the entire premium for such group insurance programs commencing with the beginning of the retirement (see District Website, Human Resources for specific coverage available). The teacher shall be responsible for paying the monthly premium amounts in advance and on such dates as determined by the District/third party administrator. The right to continue participation in such group insurance programs will discontinue upon the failure of the teacher to pay the premiums to the District/third party administrator, or the expiration of insurance availability under the insurance policy provisions. Since long-term disability insurance coverage replaces salary, and a retiree receives no salary, long-term disability insurance coverage is not available.

  • Sickness and Accident Insurance During the term of this Agreement, the Employer shall pay the required premiums for each employee for sickness and accident insurance which will pay Three Hundred Fifty Dollars ($350.00) per week for a period of fifty-two (52) weeks. This benefit shall be payable from the thirty-first (31st) day of disability due to illness or injury.

  • Employment Insurance ‌ Employment insurance coverage will be provided during the life of this agreement for regular and auxiliary employees who would, if employed by a private employer, be eligible for such coverage under the provisions of the Employment Insurance Act.

  • Contractor Insurance 2.1 The awarded Vendor shall furnish the Owners with a Certificate of Insurance XXXXX and associated endorsements in the kinds and minimum amounts as detailed in the attached "Insurance Requirements for all Contracts" at time of award.

  • Employment Insurance Rebate The short-term sick leave plan shall be registered with the Employment Insurance Commission (EIC). The nurses' share of the employer's Employment Insurance premium reduction will be retained by the Hospital towards offsetting the cost of the benefit improvements contained in this agreement.
