Notice and evidence. (a) An employee is required to inform their employer that they will be absent from work due to personal illness or injury. The notice must be given as soon as reasonably practicable unless the circumstances are beyond the employee’s control.

The employee shall be required to provide a medical certificate for any single days absences in excess of two single days in any 12 month period; and ▪ If an employee takes time off for personal/carer’s leave on the day before or after: a public holiday; a rostered day off (RDO); annual leave; or long service leave they are to provide to the Employer, upon request, with a medical certificate or statutory declaration. For any period of xxxxx’s leave the employee must provide documentary such as a medical certificate or a statutory declaration in relation to the member being cared for.
