The employee shall be required to provide a medical certificate for any single days absences in excess of two single days in any 12 month period; and ▪ If an employee takes time off for personal/carer’s leave on the day before or after: a public holiday; a rostered day off (RDO); annual leave; or long service leave they are to provide to the Employer, upon request, with a medical certificate or statutory declaration. For any period of xxxxx’s leave the employee must provide documentary such as a medical certificate or a statutory declaration in relation to the member being cared for.
