Grievance Form. The grievance form which must be used for filing of grievances shall be provided by the District. Such form shall be readily accessible in all school buildings or electronically available on the District’s website. (See Attachment B – Grievance Form)

Grievance Form. At every LEVEL, the grievant and management’s representative shall complete the Grievance Form, attached hereto, marked Appendix E. Failure by the grievant to complete the Grievance Form within the time limits set forth above at each relevant LEVEL shall be considered a waiver of the Grievance.

Grievance Form. All grievances shall be filed promptly and, in order to be processed beyond Step 1, shall be reduced to writing on a form provided by the Office. The written grievance form, which shall be signed by the aggrieved employee, shall include at least the date the grievance arose, the facts involved, the section of the collective bargaining agreement allegedly violated and the relief sought.
Grievance Form. The Union shall furnish a grievance form. This form shall be used in filing a grievance. One (1) copy of the form shall be the property of the employee filing the grievance.
