Computer Vision Care. 162. In addition to any vision plan contained as part of the health benefits referred to above, beginning with the benefit plan year effective January 1, 2013, all represented employees, who are Health Service System members shall be eligible for one (1) annual Computer Vision Care examination and prescribed eyewear as defined under the Health Service System of San Francisco Vision Service Plan (VSP) Vision Care “VDT Benefit,” in addition to the vision care benefits provided in the standard VSP plan provided. For reference, this additional plan is the same plan provided to CCSF Local 21 represented employees.

Computer Vision Care. (CVC) PLAN The City of Los Angeles and the American Federation of State, County and Municipal Employees (AFSCME), AFL-CIO recognizes that employees of this Unit who operate computer equipment or other digital devices should have a complete eye exam by an optometrist who can detect Computer Vision Syndrome symptoms, such as neck and shoulder pain, headaches, fatigue, irritated eyes, blurred or double vision and loss of focus through a Computer Vision Care eye exam. The Union shall contract with a Computer Vision Care Optical Plan provider. The service provider shall employ qualified professional staff to provide an optometric exam containing elements as agreed by the provider: The service provider shall also provide to each employee the appropriate computer vision eyewear, when necessary. The Union shall have the responsibility to ensure that only the employees of this Unit who operate City CVC equipment shall be eligible for this optometric exam and the computer vision glasses. Each eligible employee may be examined by the service provider only once every two years. The Union shall also have the responsibility to notify the employees of this Unit as to the availability of the CVC Plan. The Union may expend a portion of the funds which are in the CVC Trust Fund to purchase for CVC ergonomic equipment for employees of this Unit on an as needed basis, as determined by ergonomic evaluation. The ergonomic evaluation shall be conducted by the Personnel Department. The Union may expend a portion of the funds in the CVC Plan to conduct an ergonomic training course, should one not be available through the Personnel Department. The training will be conducted on City time, subject to approval of the supervisor. The parties agree to reopen this article during the term of this MOU, if necessary, to discuss additional funding for the plan. The CVC Optical Plan shall perform an annual audit of its expenditures, to be conducted by an independent qualified CPA firm. The CVC Plan shall provide copies of said audit report to the City Administrative Officer. The CVC Plan shall also provide an annual report listing the following information:

  • Vision Care Effective July 1, 2000, the District shall provide all full-time employees and their dependents with Vision Service Plan (VSP) Plan C. This plan shall provide for a comprehensive exam and new lenses every 12 months, and new frames every 12 months. All other services will be pursuant to the standard VSP plan description, except that it will reimburse up to $50 for examinations by non-panel providers. There shall be a $10 annual deductible on materials only. In addition, the following vision plan enhancements shall take place effective July 1, 2000: $60 wholesale frame allowance; computer glasses; progressive lenses, tints, and UV coatings.

  • Vision Care Plan The County agrees to provide a Vision Care Plan for all employees and dependents. The Plan will be the Vision Service Plan - Plan A with benefits at 12/12/24 month intervals and with twenty dollar ($20.00) deductible for examinations and twenty dollar ($20.00) deductible for materials. The County will fully pay the monthly premium for the employee and dependents and pick up inflationary costs during the term of the Agreement.

  • Vision Care Benefits (a) The Employer shall provide each regular, full-time employee (and his eligible dependents*) the Blue Cross/ Blue Shield of Michigan Vision A-80 Revised Plan, subject to such conditions, exclusions, limitations, deductibles and other provisions pertaining to coverage as stated in said plan. The Employer shall pay 95% of the illustrated premium cost of such benefit and the employee shall pay the balance.

  • Mobility The ability to move indoors from room to room on level surfaces at the normal place of residence.

  • Special Service networks The following services must be received from special service network providers in order to be covered. All terms and conditions outlined in the Summary of Benefits apply.
