Accident/Incident Report. PROVIDER agrees to submit a written accident report to BOARD within five (5) days of an accident or incident when a pupil has suffered an injury, or injured another individual, or has been involved in an activity requiring notification of law enforcement or emergency personnel.
Accident/Incident Report. XXXXXXXXXX agrees to give immediate verbal notice and submit a written report to LEA within five (5) days of any serious injury to a pupil. Serious injury means an injury requiring notification of law enforcement or emergency Personnel.

The Contractor must immediately notify the Supervisor of the works appointed by ENEL the occurrence of any incident or accident involving death, injury, damage to property, damage to the environment, or exposure to a hazardous substance, and send a complete written report, within the twenty-four (24) hours following the event, stating the site, date and time when it occurred, name of the person(s) affected, injuries suffered, probable causes, damage caused, measures taken, measures to be taken , and any other information it may consider necessary for the knowledge of the facts. The Supplier shall give immediate notice to the Supervisor of the works designated by ENEL whenever any incident or accident should occur that involves death, injury, property damage, environmental damage or exposure to a dangerous substance and submit in writing, within twenty-four (24) hours following the event, a full report indicating the place, day and time it occurred, name of the affected persons, injuries suffered, probable causes, damages caused, actions taken, actions to be taken and all the information considered necessary for the knowledge of the circumstances. This report does not release the Supplier from any of the responsibilities pertaining thereto. In addition, the Supplier must report the occurrence of any accidents to the Government Entities in accordance with the applicable Local Laws and Regulations.
Accident/Incident Report. The PAH shall report immediately any major accident/incident (including fire) resulting in any one or more of the following: causing one or more fatalities or one or more disabling injuries; damage of Government property exceeding $10,000; affecting program planning or production schedules; degrading the safety of equipment under a project, such as personnel injury or property damage may be involved; identifying a potential hazard requiring corrective action. The PAH shall prepare the report (XX-XXXX-81563) for each incident. Material Safety Data Sheets (MSDS): The PAH shall prepare and maintain MSDS for all materials used and generated under this Agreement. Environmental Requirements include the following: Pollution Prevention: Consideration should be given to alternative materials and processes in order to eliminate, reduce, or minimize hazardous waste being generated. This is to be accomplished while minimizing item cost and risk to item performance. Environmental Compliance: All activities must be in compliance with Federal, State, and local environmental laws and regulations, Executive orders, treaties, and agreements. The PAH shall evaluate the environmental consequences and identify the specific types and amounts of hazardous waste being generated during the conduct of efforts undertaken under this Agreement.
Accident/Incident Report. Contractor agrees to submit a written accident report to LEA within 24 hours of incident when a pupil has suffered an injury requiring medical attention provided by a medical practitioner resulting from physical restraint, injured another individual, is named as a suspected perpetrator or victim in an official sexual harassment complaint, or has been involved in an activity requiring notification of law enforcement or emergency personnel.

PROVIDER must train all tutoring personnel in appropriate procedures for handling and reporting accidents or incidences when a pupil has suffered an injury, injured another individual or has been involved in an activity requiring notification of law enforcement or emergency personnel.

Accident/Incident Report. The SUPERINTENDENT shall submit immediately (within 24 hours) electronically and mail an accident or incident reports to DISTRICT when it becomes aware of circumstances including, but not limited to: allegations of molestation, child abuse, missing children under SUPERINTENDENT’S supervision, the need for mental health services, injuries requiring medical attention, injuries resulting from physical restraint, Behavioral Emergency Reports, student has injured another individual, or student has been in an activity requiring notification of law enforcement or emergency personnel.
