Accident/Incident Report. PROVIDER agrees to submit a written accident report to BOARD within five (5) days of an accident or incident when a pupil has suffered an injury, or injured another individual, or has been involved in an activity requiring notification of law enforcement or emergency personnel.

Accident/Incident Report. XXXXXXXXXX agrees to give immediate verbal notice and submit a written report to LEA within five (5) days of any serious injury to a pupil. Serious injury means an injury requiring notification of law enforcement or emergency Personnel.
Accident/Incident Report. The Contractor must immediately notify the Supervisor of the works appointed by ENEL the occurrence of any incident or accident involving death, injury, damage to property, damage to the environment, or exposure to a hazardous substance, and send a complete written report, within the twenty-four (24) hours following the event, stating the site, date and time when it occurred, name of the person(s) affected, injuries suffered, probable causes, damage caused, measures taken, measures to be taken , and any other information it may consider necessary for the knowledge of the facts. Environmental Requirements include the following: Pollution Prevention: Consideration should be given to alternative materials and processes in order to eliminate, reduce, or minimize hazardous waste being generated. This is to be accomplished while minimizing item cost and risk to item performance. Environmental Compliance: All activities must be in compliance with Federal, State, and local environmental laws and regulations, Executive orders, treaties, and agreements. The PAH shall evaluate the environmental consequences and identify the specific types and amounts of hazardous waste being generated during the conduct of efforts undertaken under this Agreement.
