Suspension and Termination of Service. You understand and acknowledge that your VOIP product requires a broadband connection to operate. Should any of your services be suspended or terminated for any reason, including for failure to make timely payment or due to usage in violation of the applicable AUP, your ability to use your VOIP line, including to contact 911/E911, will be disabled. Acknowledgement and warning labels. By using our VOIP services, you acknowledge that you are responsible for notifying all potential users who may place calls using your VOIP of the 911 limitations described herein. You are responsible for any notices of the potential lack of 911/E911 operation to any users of your VOIP service regardless of whether or not you have properly labelled your equipment. Non-voice systems. You acknowledge that the services are not set up to function with non-voice services such as home security systems, medical monitoring equipment, TTY equipment, or entertainment or satellite television systems. We are not liable for interruption or disruption of such systems by the services. 911/E911 limitation of liability and indemnity. In addition to limits of liability and indemnification that may be contained elsewhere in this Agreement, you agree that we are not liable for any service outage or inability to dial 911 or any other emergency telephone number due to the conditions described in this section. You agree to defend, indemnify and hold harmless us, our owners, managers, officers, directors, employees, affiliates and agents, and any other service provider who furnishes services to you or a user in connection with the services, from any and all claims, actions, losses, damages, fines, penalties, costs and expenses (including, without limitation, attorneys’ fees) by, or on behalf of, us or any third party or user of the service relating to 911 dialing or the inability to access or reach emergency 911 services.

Suspension and Termination of Service. SEE Link may terminate and/or suspend the Executing Broker’s license grant and/or suspend, terminate or limit any of the Executing Broker’s use of the Service without liability, giving at least prior verbal notice, based on SEE Link’s reasonable belief that: (i) the Service is being used in breach of the provisions of this Agreement or otherwise in a potentially harmful or unlawful manner; (ii) the use of the Service adversely affects SEE Link’s (or its suppliers’) equipment, security network infrastructure or its service to others; or (iii) the Executing Broker fails to pay undisputed charges for the Service after being given notice. In the event the Service is suspended, SEE Link will use commercially reasonable efforts to inform the Executing Broker and will work with the Executing Broker to resolve such issues and re-instate the Service. Furthermore, SEE Link shall suspend or otherwise limit Executing Broker’s right to receive the Service, without liability for SEE Link, if SEE Link notified by the Local Exchange that the Executing Broker’s membership in the Local Exchange has been suspended or its right to trade on the Local Exchange has been otherwise limited. The Executing Broker acknowledges and agrees that SEE Link shall be entitled to suspend or terminate the Service that the Executing Broker receives with respect to a particular Contracted Originating Broker, if SEE Link suspends or terminates the Membership Agreement it has concluded with such Contracted Originating Broker or if such agreement is terminated by such Contracted Originating Broker. In such case, SEE Link shall have no liability towards the Executing Broker for the inability to receive the Service with respect to such Contracted Originating Broker. Furthermore, the Executing Broker further acknowledges and agrees that SEE Link shall be entitled to terminate the Service that the Executing Broker receives with respect to a particular Contracted Originating Broker with reasonable prior notice, if SEE Link suspends or terminates the Participation Agreement it has concluded with the relevant Participating Exchange that has admitted the relevant Contracted Ordering Broker as its member. In such case, SEE Link shall have no liability towards the Executing Broker for the inability to receive the Service with respect to such Contracted Originating Broker. Suspension and Termination of Service. The decision to remove a PERS is at the sole discretion of the Inclusa team. For all PERS removals, notification will be by telephone, followed by written notification, from the authorized Inclusa team. If the provider is notified directly by a member’s family or other representative to remove the PERS, authorization must first be obtained from the Inclusa team. When a member with a PERS no longer requires such services, regardless of the reason, the Inclusa team will discuss with appropriate staff, as needed, and contact the provider, so that the PERS may be transferred or removed. 1. When a member’s services are suspended because of the member’s admission to the hospital, the Inclusa team will notify and/or authorize the provider to take the unit off-line. Services will be resumed to the member only after the Inclusa team notifies the provider. Payment for leased equipment will be made at the standard unit price as long as a unit remains in the home of a member. 2. The provider shall disconnect/remove a PERS from a member’s residence within 5 working days of notification by the Inclusa team. Inclusa will discontinue payment effective 5 days after notice of disconnect or the following day after removal of the PERS, whichever is sooner. 3. If PERS is discontinued due to member disenrollment or death, payment will cease the following day after disenrollment or death. 4. Inclusa is not responsible for equipment that the provider is unable to collect from the member or their representative after service is terminated.
Suspension and Termination of Service. In the event you wish to discontinue usage of the Bill Payment Service, you must contact our Member Service Department at least ten (10) Business Days prior to the actual service discontinuation date. We reserve the right to temporarily suspend the Bill Payment Service in the event that there are insufficient funds in your Draw Account to pay your scheduled bill payments.

Customer shall be fully responsible for payment to Carrier for the full amount of invoice including without limitation any fraudulent or unauthorized use by Customer, any federal, state, county, local or other governmental taxes, fees or duties now or hereafter imposed on the sale or use of the Services.
