Monthly Premium Payments. For the initial term of this Group Agreement, the Group shall submit to KFHPWA for each Member the monthly premiums set forth in the current Premium Schedule and a verification of enrollment. Payment must be received on or before the due date and is subject to a grace period of 10 days. Premiums are subject to change by KFHPWA upon 30 days written notice. Premium rates will be revised as a part of the annual renewal process. KFHPWA reserves the right to re-rate this benefit package if the demographic characteristics change by more than 15%. Rate Stabilization Reserve – Refundable KFHPWA will establish a Rate Stabilization Reserve (RSR) for the Group that is based upon the financial performance of this Group Agreement and all other Group Agreements in effect between KFHPWA and the Group.

Monthly Premium Payments. For the initial term of this Agreement, the Group shall submit to GHO for each Participating Party the monthly premiums set forth in the current Premium Schedule and a verification of enrollment. Payment must be received on or before the due date, and is subject to a grace period of ten (10) days. Premiums are subject to change by GHO upon thirty (30) days written notice. Premium rates will be revised as a part of the annual renewal process. Except for services for hospitalization as set forth in Section III.C., the Group will be responsible for costs of any claims incurred or services provided after retroactive termination of this Agreement or retroactive termination of a Participating Party in the Group because of non-payment of premium. The Group will be responsible for the cost of any claims incurred for services paid or provided in reliance on inaccurate reporting of member enrollment made by the Group. In the event the Group increases or decreases enrollment at least twenty-five percent (25%) or more, GHO reserves the right to require re-rating of the Group. If agreed to in advance, responsibility under this Monthly Premium Payments provision may be assigned to each Participating Party. Such assignment will be noted in the Group Master Application attached to and incorporated into this Agreement.

This continued coverage will apply only in the event the Dependent(s) were enrolled under the agreement at the time of the Subscriber’s death. This 12 month period will apply toward the COBRA continuation period, if elected, and will not extend any other continuation benefits the Group may offer. Surviving Dependents who become ineligible for coverage during the 12-month extension of benefits following the Subscriber’s death may apply for COBRA and the premium waiver will no longer apply.
Monthly Premium Payments. (e) CONTRACTOR will be required to provide in a timely manner financial and statistical information necessary in the capitation rate determination process. Encounter data provided by CONTRACTOR must conform to all HHSC requirements. Encounter data containing non-compliant information, including, but not limited to, inaccurate member identification numbers, inaccurate provider identification numbers, or diagnosis or procedure codes insufficient to adequately describe the diagnosis or medical procedure performed, will not be considered in the CONTRACTOR'S experience for rate-setting purposes. Information or data, including complete and accurate encounter data, as requested by HHSC for rate-setting purposes, must be provided by CONTRACTOR to HHSC: (1) within thirty (30) days of receipt of the letter from HHSC requesting the information or data; and (2) no later than March 31st of each year.
