Premium Payment. 14.01 (a) (Article 14.01(a) applies to full-time nurses only) If a nurse is authorized to work in excess of the hours referred to in Article 13.01 (a) or (c), she or he shall receive overtime premium of one and one-half (1 1/2) times her or his regular straight time hourly rate. Notwithstanding the foregoing, no overtime premium shall be paid for a period of less than fifteen (15) minutes of overtime work where the nurse is engaged in reporting functions at the end of her or his normal daily tour. If authorized overtime amounts to fifteen (15) minutes or more, overtime premium shall be paid for the total period in excess of the normal daily tour. Overtime premium will not be duplicated for the same hours worked under Article 13.01 (a) and (c) nor shall there be any pyramiding with respect to any other premiums payable under the provisions of this Collective Agreement. Nothing herein will disentitle the nurse to payment of the normal tour differential provided herein. For purpose of clarity, a nurse who is required to work on her or his scheduled day off shall receive overtime premium of one and one-half (1 1/2) times her or his regular straight time hourly rate except on a paid holiday the nurse shall receive two (2) times her or his straight time hourly rate The Hospital agrees that if the Collective Agreement provided a greater overtime premium for overtime work immediately prior to this Agreement, the Hospital will continue to pay such greater overtime premium. This is not intended to entitle the nurse to be paid for work performed while engaged in the reporting functions as provided herein.

Premium Payment. 12.1 The first premium for this insurance is due from the commencement date of the policy. Each and every subsequent premium is payable on the premium due date. For insurance with periodic premium payment per month or year, the premium is automatically debited at the beginning of the month from the specified IBAN bank account.
12.2 If, for whatever reason, the follow-up premium is not paid on time, there is, after a reminder, still one month to pay it. If the subsequent premium including costs is not paid to the administrator in full within the said period, the cover is suspended automatically from the premium due date until the premium arrears are paid to the administrator in full. An event during the period that the cover is suspended and that results in entitlement to a benefit on the basis of this insurance is not covered. Moreover, the insurer shall be entitled to terminate the insurance. Already paid premiums shall not be repaid. In case of a pledged policy, the insurer will inform the finance company of the lapse of the policy.
12.3 Any and all judicial and extrajudicial costs that are incurred by the insurer for the collection of overdue premium instalments and possible statutory interest due shall be borne by the policyholder.

CDN must receive your first premium from your employer within 45 days after you enroll in Cal-COBRA. This first premium covers the time from the date your coverage ended because of the qualifying event up to the day You signed up for Cal- COBRA. You must then pay a monthly premium as instructed by your employer as long as you stay on Cal-COBRA. If your former employer stops offering CDN when you are on Cal-COBRA: • You can elect/enroll in Cal-COBRA with the new health plan offered by your employer. • You must enroll and pay your first premium as instructed by your employer with the new health plan no more than 30 days after You receive notice that CDN is no longer being offered. If you do not meet this deadline, your Cal-COBRA Benefits end. You will lose Cal-COBRA if: • You do not pay your premiums on time. • You move outside the CDN Service Area. • Your former employer no longer offers any health plan. • You sign up for or become eligible for Medicare. • You sign up for another health plan. (However, if your new plan has a waiting period for pre-existing conditions and you have not used up all of your Cal- COBRA, You can keep your Cal-COBRA until the waiting period is over.) • You commit fraud, which means that you intentionally deceive CDN or you misrepresent yourself or allow someone else to do so in order to get health care services.
