HOSPITALIZATION/MEDICAL INSURANCE COVERAGE. (a) The Employer will pay the premiums for the following dental, hospi talization, and medical insurance coverage for the employee and his family: MEBS (Michigan Employee Benefit Services) PPO 1, with $10/20 drug, dental, vision and hearing riders.

HOSPITALIZATION/MEDICAL INSURANCE COVERAGE. The hospitalization and medical insurance programs in effect when this Agreement is ratified shall be continued during the term of this Agreement. The Village will continue to offer a basic health insurance plan with the HMO, as well as a PPO and HDPPO as the Village’s supplemental health insurance plans requiring a premium contribution by the employee (as listed below), co-pays, deductibles, reimbursement for being out of network, out of pocket maximum for being out of network, etc. Employees may select single, single plus spouse, single plus child(ren) or family coverage in one of the health programs offered by the Village during the enrollment period established by the Village. The insurance plan year typically commences on January 1st of each year. The Village shall provide group health insurance benefits to employees, with such benefits to be provided in the group insurance policy(s) applicable to all Village employees at the rates assessed under such policy(s), which the Village shall enter from time to time.
HOSPITALIZATION/MEDICAL INSURANCE COVERAGE. The Hospitalization/Medical/Dental/Optical package shall be changed to the Blue Cross Blue Shield (BCBS) of Michigan. In the event (BCBS) does not provide comparable benefits to the previous BCBS Plan the Union may petition the Employer to seek an alternate comparable Hospitalization/Medical/Dental/Optical package. The Employer will provide and pay the full premium cost of Dental and Optical Benefits as agreed upon between the Employer and Union. The employee must agree to the full Hospitalization/Medical/Dental/Optical package. Package is not to be divided or offered as separate plans. The Employer, after discussion with the Union, reserves the right to change the plan provider as a cost saving measure provided benefits remain comparable to those currently provided. Regular full time employees will be eligible for those benefits provided for in the BCBS Insurance of Michigan Plan or an agreed upon health insurance plan between the employer and employees. The employer will provide and pay the full premium cost of Dental and Optical Benefits. During the term of this agreement the Menominee County Board will determine its contribution towards the cost of medical benefits and prescription drug benefits using either Hard Caps, eighty percent (80%) or County Board action to exempt itself from those requirements set forth in SB7, PA 152. All premium cost above those remitted by the Employer will be paid by the participating employees through bi-weekly payroll deduction. During the term of this Agreement should the County Board act to exempt itself from the requirements in SB7, PA 152, participating employees will pay 12.5% of the premiums cost of the medical benefits premiums.

  • INDUSTRIAL INSURANCE COVERAGE The Contractor shall comply with the provisions of Title 51 RCW, Industrial Insurance. If the Contractor fails to provide industrial insurance coverage or fails to pay premiums or penalties on behalf of its employees, as may be required by law, Agency may collect from the Contractor the full amount payable to the Industrial Insurance accident fund. The Agency may deduct the amount owed by the Contractor to the accident fund from the amount payable to the Contractor by the Agency under this contract, and transmit the deducted amount to the Department of Labor and Industries, (L&I) Division of Insurance Services. This provision does not waive any of L&I’s rights to collect from the Contractor.

  • Insurance Coverages The Contractor shall procure and maintain, at its sole cost and expense, in a form and content satisfactory to City, during the entire term of this Agreement including any extension thereof, the following policies of insurance which shall cover all elected and appointed officers, employees and agents of City:

  • Retiree Medical Insurance Retiree insurance coverage is included within each medical plan for all retirees under the age of 65 years, through self-payment. The Employer shall make available an appropriate medical plan for all eligible retirees ages 65 years or older.

  • Subcontractor Insurance Coverage Contractor shall require and verify that all subcontractors maintain insurance coverage that meets the minimum scope and limits of insurance coverage specified in this Exhibit C. EXHIBIT D

  • Health and Hospitalization Insurance Single Coverage: The School District shall contribute a sum not to exceed $284.00 per month toward the premium for individual coverage for each full-time employee employed by the School District who qualifies for and is enrolled in single coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.

  • DISABILITY INSURANCE PLAN Management shall expend for active employees of this unit who are members of LACERS the sum necessary to cover the cost of a basic disability insurance plan. Management shall also maintain a Supplemental Disability Insurance Plan, enrollment in which is at the discretion of each employee. The full cost of the Supplemental Disability Insurance Plan premiums shall be paid by the individual employees who enroll in the plan. Effective 1/1/2012 employees hired before 9/1/11 shall pay 10% of the illustrated rate for the health and dental coverage they select. Effective the first pay period after 9/1/2011 or as soon as is practicable, for employees hired on or after 9/1/11, employees shall contribute 20% of the illustrated rate for the coverage the employee selects.
