HEALTH & WELFARE PROGRAM. The Employer will provide a Group Health and Welfare Benefit Plan for employees covered under this preamble. A general description of the Health and Welfare benefits to be provided, terms and conditions, are as follows in B-1 to B-4. Employees and the Union will be provided with details of the Plan. For greater specifics relating to each benefit, the Plan text should be referred to. Unless otherwise specifically stated, all of the benefits shall apply to all employees in the bargaining unit who work at least twenty-five (25) hours per week. Employees may opt out of the Dental and Extended Health Care coverage only, if they are receiving coverage from another Plan. Both the Employer and the Employee shall share in the bi-weekly cost of Health and Dental benefits, with the Employer paying seventy (70%) percent of the cost of the premiums and the employees paying thirty (30%) percent of the cost of the premiums for all Health and Welfare benefits referred to in this appendix, in effect at the date of ratification and thereafter, unless otherwise specifically stated. The Employer shall have the right to make arrangements for the replacement of such benefits provided that benefit levels are maintained or improved and do not increase costs to the employees. Under such circumstances, the Employer shall first provide the Union with advance notification in writing, detailing the specific changes that are to take place as well as the reasons for said changes

HEALTH & WELFARE PROGRAM. 35.01 The Employer agrees to pay one hundred percent (100%) of the billing rate of billing received of the Ontario Health Insurance Plan (OHIP) for all employees eligible under the Plan.
HEALTH & WELFARE PROGRAM. 42.01 The Employer agrees to pay one hundred percent (100%) of the billing rate of billing received of the Ontario Health Insurance Plan (OHIP) for all employees eligible under the Plan. It is agreed that this provision is currently inapplicable and therefore has no effect on the parties. Should there be a future legislative reinstatement of the former billing rate under the Ontario Health Insurance Plan (OHIP), the parties further agree that this provision will be effective again and its obligations applicable to the parties.

  • HEALTH & WELFARE 16:1 The parties signatory hereto shall enter into a Health and Welfare Plan for which there is a Trust Agreement, known as the Line Construction Benefit Fund, for the purpose of providing insurance benefits for eligible employees and/or their dependents. Effective the first of the month following the signature date of this Agreement, the Employer shall pay to the Line Construction Benefit Fund the sum of $6.50 for each hour worked. Hours worked shall be deemed to include straight-time hours worked, overtime hours worked, and report time not worked. Remittance shall be forwarded to the place designated by the parties hereto on or before the fifteenth (15th) day of each month for each hour worked in weekly payroll periods ending during the preceding month, together with a monthly payroll report on a form to be furnished to the Employer. It is understood and intended by the parties to this Agreement that the purpose of this clause is to establish an Employer financed Health and Welfare Trust and that contributions thereto shall not be deemed to be wages to which any employee shall have any right other than the right to have such contributions paid over to the Trust fund in accordance herewith. Failure of an individual Employer to make all payments provided for, including liquidated damages for late payments, within the time specified, shall be a breach of this Agreement and will further require action by the Trustees as set forth in the Trust Agreement. Any increase in the required contributions set forth above will be paid equally (50% by the Employer and 50% by the Employee). The amount paid by the Employee will come from their NEAP contribution.

  • HEALTH AND WELFARE PLAN 16.01 The Employer agrees to pay the amount as set out in the Wage Schedules for all hours worked for each employee towards the Insurance Plan administered by the CLAC Health and Welfare Trust Fund.

  • Extended Health Benefit Plan (a) All regular and probationary employees after three (3) months employment will be covered by a one hundred percent (100%) Extended Health Benefit Plan with the standard $100.00 deductible. The City will pay eighty percent (80%) of the costs and the twenty percent (20%) deduction for employees shall be made through payroll deductions. The extended health lifetime maximum will be $1,000,000.
