Head of Medicines Management. Date …………………………………………………….. ………………… Issue Date: May 2008 Review Date: May 2009 Lead: Head of Medicines Management APPENDIX 1 (EHC) RECORD KEEPING AND DISPENSING OF LEVONELLE 1500 THE CONSULTATION MUST BE FULLY DOCUMENTED IN THE CLIENT’S FAMILY PLANNING NOTES AS APPROPRIATE AND SIGNED BY THE DISPENSING NURSE OR PHARMACIST The nurse or pharmacist should document the following: • Date of last menstrual period • Average cycle lengthDay of cycle of unprotected sexual intercourse • Any other episodes of unprotected sexual intercourse in that cycle • Method of contraception used (if any) If the nurse or pharmacist is satisfied that there are no contraindications to the method and that all criteria have been met, then she / he should ensure; • That the patient understands the method and its efficacy • That the patient is referred to a Doctor if an IUD would be more appropriate or she would prefer one • That she is given written instructions and Levonelle 1500 patient information leaflet as well as verbal instructions • That she is aware what action to take if she vomits within 2 hours of taking the dose of Levonelle 1500 • That future contraception is discussed and issued as appropriate • That she is advised to return promptly, particularly if she has lower abdominal pain or her next bleed causes her concern or is absent The nurse or pharmacist must ensure the emergency tablet issued is labelled with the clients name, clinic or pharmacy name, date of issue and check that the expiry date has not been passed. Storage of tablets should be in a locked cupboard or drawer if in a nurse base. A record of dispensing must be kept in each area where supply is made and this must include date of issue, name of client and, where appropriate, name of nurse or pharmacist. Patients must be asked to report any adverse reaction to the issuing clinician ASAP. APPENDIX 2 (EHC) FRASER GUIDELINES A nurse or pharmacist can give contraceptive advice and treatment to young people under 16 without parental consent provided that she /he is satisfied that ALL the following requirements are fulfilled: • The young person understands the advice • The nurse or pharmacist cannot persuade the young person to inform her parents or allow the nurse or pharmacist to inform the parents that she is seeking advice on contraception • The young person is very likely to begin or will continue to have sexual intercourse with or without contraceptive treatment • Unless she receives contraceptive advice or ...

  • Patch Management All workstations, laptops and other systems that process and/or 22 store PHI COUNTY discloses to CONTRACTOR or CONTRACTOR creates, receives, maintains, or 23 transmits on behalf of COUNTY must have critical security patches applied, with system reboot if 24 necessary. There must be a documented patch management process which determines installation 25 timeframe based on risk assessment and vendor recommendations. At a maximum, all applicable 26 patches must be installed within thirty (30) calendar or business days of vendor release. Applications 27 and systems that cannot be patched due to operational reasons must have compensatory controls 28 implemented to minimize risk, where possible.
