Custodian of Records. At the request of UPS, You shall make available a custodian of records to testify, at UPS’s expense, to a My Choice Enrollee’s assent to enrollment in the UPS My Choice® service using all available information.
Custodian of Records. From and after Closing, Seller shall remain as custodian of medical records of Seller, including all medical records related to the Business prior to the Closing. PC Buyer shall not assume any responsibility, and shall not be liable, to any third party related to Seller’s medical records or protected health information. PC Buyer may examine, and at its expense, make copies of medical records as necessary in connection with the provision of medical services after the Closing. Seller shall provide PC Buyer with reasonable access to such medical records during normal business hours, upon reasonable advance notice, to examine and make copies of the medical records, as necessary, to conduct the Business after Closing. Seller’s medical records, including all medical records related to the Business prior to Closing, shall be Excluded Assets (as defined in Section 1.2).
Custodian of Records. The documents and materials that constitute the record of proceedings on which these findings have been based are located at the City of Ontario City Hall, 000 Xxxx “X” Xxxxxx, Xxxxxxx, Xxxxxxxxxx 00000. The custodian for these records is the City Clerk of the City of Ontario.
Custodian of Records. Employee will ensure that the records of the organization are maintained as required by law and made available when required by authorized persons. These records may include founding documents, (eg. letters patent, articles of incorporation), lists of directors, board and committee meeting minutes financial reports, and other official records.
Custodian of Records. Each Participating Entity will serve as their individual custodian for purposes of election records as required by law. Each Participating Entity shall appoint a qualified person to act as Custodian of Records for the Entity to perform the duties imposed by the Election Code on the custodian of records for its respective entity.
Custodian of Records. The Contracting Officer will serve as the custodian of voted ballots as set forth in Section II.P. above. The District will be responsible for having a District representative serve as the custodian of all other election records.
Custodian of Records. This Contract is subject to the requirements of the Colorado Open Records Act, §§ 24-72-200.1 et seq., C.R.S. ("XXXX"). Subject to the requirements of XXXX, XXXX agrees not to disclose, publicly release, or produce for any purpose, including in response to a subpoena or other court or governmental order, PROVIDER’s Confidential Information without giving PROVIDER as much written notice as is reasonably practicable, and an opportunity to object to the disclosure or production of any Confidential Information. At the request of SIPA, PROVIDER shall provide such reasonable assistance as may be requested by SIPA to comply with this provision. PROVIDER shall designate in writing any records that may not be disclosed and the grounds for non-disclosure according to XXXX. For the purposes of the Colorado Open Records Act, each EGE CUSTOMER shall, at all times, be the custodian of EGE CUSTOMER records. If SIPA is the CUSTOMER, SIPA shall, at all times, be the custodian of SIPA records. Neither SIPA nor PROVIDER shall be deemed either the custodian of EGE CUSTOMER records, nor the custodian’s agent.
Custodian of Records. The AGENCY participating in the Clearinghouse shall re- confirm the identity of their Custodian of Record(s) by July 1 of each year and must notify BCOE in writing whenever changes occur throughout the year. A copy of the AGENCY employee’s Custodian of Record confirmation letter must also be furnished to BCOE for official recordkeeping. The AGENCY shall identify its Custodian of Records(s) on the attached Exhibit “C”.
Custodian of Records. Custodian of Records. The Sheriff’s Office will be considered the “Custodian of Record” regarding any audio recording of radio or telephone conversations recorded by BCCC.
