Certificate of Health. Participant will submit a Certificate of Health, signed by a physician, at any time requested throughout her participation in the Programs.
Certificate of Health. When a member has been absent for more than five (5) but less than ten (10) consecutive work days because of personal illness, a Certificate of Health form shall be filed with the Department of Human Resources immediately upon the member’s return to work. In the event that the absence is for ten (10) days or more, a Certificate of Health is to be filed at the end of each payroll period. Failure to file the Certificate of Health will result in delay of compensation for accumulated sick days. The Certificate of Health shall be signed by the member and his/her physician and shall list the name and address of the attending physician, the dates the physician was consulted, and a statement by the physician that member was unable to work. Nothing in this form shall be construed to waive the physician-patient privilege.

Certificate of Health. The Noble Public Schools will pay for costs for a reasonable and customary physical and the CDL license for all certified employees whose duties include driving for the Noble district.
Certificate of Health. As a condition to entering or continuing employment, the Teacher is required to submit a Certificate of Health signed by a licensed physician as provided by K.S.A. 72-5213.
Certificate of Health. If requested by the employer, the employee shall provide a certificate of health from a physician appointed by the employer prior to the start of employment. The employer shall pay the costs of the certificate.
Certificate of Health. An employee who used more than his/her annual allowance of leave and thus receives pay for accumulated leave may be asked to present a doctor’s certificate certifying the necessity of this action.
