Relevant PHI definition

Relevant PHI means the Personal Health Information listed in Appendix 2 hereto, titled “Relevant PHI”.
Relevant PHI means personal health information (as defined in the Personal Health Information Protection Act, 2004, S.O. 2004, c. 4) contained in OLIS, namely, laboratory results reported from and entered by Participating Laboratories, including each patient’s name, date of birth, age and sex, health number or other primary identifier, symptomatic and specimen information and test results; and
Relevant PHI means the Personal Health Information listed in Schedule D hereto entitled “Relevant PHI”

Examples of Relevant PHI in a sentence

  • The Relevant PHI is all of the information provided to OLIS that relates to an individual who, on the basis of implied consent, has consented to the disclosure of his or her Personal Health Information by the Laboratory to Ontario.

  • The primary purpose for which the Relevant PHI is disclosed to Ontario by the Laboratory under this Agreement is so that Ontario can provide eHealth Services to facilitate the provision of health care to individuals by health care providers.

  • Upon termination of this Agreement, the Recipient will have no further obligation to provide the Relevant PHI to the Province.

  • Only provide access to Relevant PHI in the Solution to the User’s End Users that require such access in order to perform their work on the User’s behalf.


More Definitions of Relevant PHI

Relevant PHI means the Personal Health Information that is collected, used, or disclosed through the Solution for the Purpose.
Relevant PHI means the PHI listed in an Appendix, as it may be amended from time to time, which is relevant to generating a BI Report. 2.0 PROVISION, AUTHORISED ACCESS AND USE 2.1 No ownership in or to BI Reports is transferred to Participant by virtue of this Schedule and Participant has only the limited right to access and use BI Reports as set out in this Schedule. 2.2 Participant acknowledges that the BI Access Services are provided to Participant solely for the access to and use of BI Reports by Participant and its Users in connection with the Permitted Purposes, and not for any other use or use by any other person. Participant will ensure that it will not permit any other use or any other person to use the BI Access Services. 2.3 Upon request by KHSC, the Participant will provide a copy of any privacy or security assessments conducted on any Participant Systems that are involved in the access to or use of the BI Access Services, including BI Reports, as soon as reasonably possible. Any such assessments will be deemed to constitute Confidential Information of Participant and may only be disclosed to thirdparties or used by KHSC to assess the privacy and security of Participant’s systems in respect of the BI Access Services.2.4 Use of BI Reports. As a condition of Participant receiving BI Access Services and of any User and/or Participant gaining access to BI Reports, the Participant agrees to be responsible for:
