Requirement to Maintain Applicable License. All employees are required to maintain applicable licenses in good standing at all time. Employees are responsible for the purchase of any and all materials educational or otherwise which are necessary to maintain and update his/her knowledge and skills as required for the successful performance of his/her job duties and responsibilities. Any employee whose license has lapsed is obligated to notify the Appointing Authority. Failure to so notify will subject the employee to disciplinary action up to and including termination.

Requirement to Maintain Applicable License. All employees are required to maintain applicable professional or other licenses in good standing at all times. Employees are responsible for the purchase of any and all materials, educational or otherwise, which are necessary to maintain and update their knowledge and skills as required for the successful performance of their job duties and responsibilities. Any employee whose license is required by statute, or is necessary to perform his/her normal job functions, has lapsed or otherwise been revoked is obligated to notify the Appointing Authority immediately. Failure to make such notification may result in disciplinary action, likely including termination. After notification that his/her license has lapsed, the employee will take all appropriate steps to renew the license. Failure to take such steps may result in disciplinary action up to and including termination. This Section does not apply to those employees whose license has been revoked or suspended due to a disability.
Requirement to Maintain Applicable License. All employees in the bargaining unit are required to maintain a driver’s license valid in Massachusetts at all times and make licenses available for periodic review. Employees are responsible for the purchase of any and all materials and tests, educational or otherwise, which are necessary to maintain and update their knowledge and skills as required for the successful performance of their job duties and responsibilities. Any employee whose license has lapsed, been revoked or is otherwise subject to conditions is obligated to notify the Athletic Director and Vice-Chancellor of Human Resources immediately. Failure to make such notification may result in disciplinary action, likely including termination. The University will periodically verify through its third party service that the required license is in effect. After notification that his/her license has lapsed, the employee will take all appropriate steps to renew the license. Failure to take such steps may result in disciplinary action up to and including termination.

  • Commercial Driver’s License If the job responsibilities of the classification of work to which an employee is regularly appointed or is assigned on an out-of-class basis involve the driving of vehicles requiring the driver to have a state Commercial Driver’s License (CDL), fees charged by the state for acquiring the license and all required endorsements shall be reimbursed by the City upon the employee having successfully attained the CDL or CDL renewal. The physical exam required to obtain or renew the license may be done on City time. The City will pay as a maximum amount, the rates charged by City identified clinics for the physical exam. Employees shall be notified of clinics offering the exam at this reimbursement rate. If an employee is covered by a City medical plan that includes coverage for physical exams, the employee shall have the exam form completed through the plan's providers (Group Health or Aetna) or shall seek reimbursement through the medical plan. Employees required to have a Hazardous Material endorsement (HME) are required per Federal regulations to submit to a background records check and fingerprinting. Employees may make application for such HME on City time and shall be reimbursed for the fees associated with the background records check and fingerprinting if such endorsement is required by the job. The City shall make a reasonable effort to make City trucks or equipment available for skill tests. In addition, for those employees qualifying as described above, fees charged for department-approved classes offered for employees to assist them in passing this exam shall be reimbursed on a one-time-only basis. Employees in other job titles or positions not involving the driving of vehicles requiring the CDL, who wish to take exam preparation or driver training courses, may request approval of the courses and reimbursement of fees in the normal manner in which educational expenses are applied for and approved by departments; provided, however, license fees for those individuals will not be reimbursed, nor shall the City be obligated to make City trucks or equipment available for skill tests for these individuals. Nothing contained herein shall guarantee that written exams, skill tests, or training classes established for the purposes described herein shall be conducted during regular work hours or through adjusted work schedules, nor shall such written exams, skill tests, or training classes be paid for on an overtime basis.

  • Staff-to-Youth Ratio Requirement For all group activities the AGENCY shall abide by the staff-to-youth ratio range that is between the ratio established by its written policy and procedures and the ratio stated in Section 402.305(4), Florida Statutes. If the staff-to-youth ratio does not meet the minimum standard of care as stated in Section 402.305 (4), Florida Statutes, the AGENCY shall increase staff- to-youth ratios to meet these minimum standards. Section 402.305(4), Florida Statutes, states the minimum staff-to-youth ratio for on-site group activities for children five (5) years of age or older there must be one (1) childcare personnel staff to every twenty-five (25) children; for field trips and other off-site activities, Chapter 65C-22.001, Florida Administrative Code, requires one (1) extra adult, in addition to the on-site requirement. This standard shall be required for all programming paid for with funds under this Contract. The AGENCY will ensure that the staffing pattern is adequate and is adjusted to meet programmatic needs. The AGENCY shall adjust its staffing ratio to meet any ratio update required by Florida Statutes that occurs during the Contract year.

  • Sales Reporting Requirements This is a requirement of the TIPS Contract and is non-negotiable. By submitting this proposal, Vendor certifies that Vendor will properly report all TIPS sales. With the exception of TIPS Automated Vendors, who have signed an exclusive agreement with TIPS regarding reporting, all TIPS Sales must be reported to TIPS by either: (1) Emailing the purchase order or similar purchase document (with Vendor’s Name, as known to TIPS, and the TIPS Contract Name and Number included) to TIPS at xxxxxx@xxxx-xxx.xxx with “Confirmation Only” in the subject line of the email within three business days of Vendor’s acceptance of the order, or; (2) Within 3 business days of the order being accepted by Vendor, Vendor must login to the TIPS Vendor Portal and successfully self-report all necessary sale information within the Vendor Portal and confirm that it shows up accurately on your current Vendor Portal statement. No other method of reporting is acceptable unless agreed to by the Parties in writing. Failure to report all sales pursuant to this provision may result in immediate cancellation of Vendor’s TIPS Contract(s) for cause at TIPS’ sole discretion.
