Life Insurance Plan. Three (3) times normal annual earnings exclusive of overtime to a maximum of $100,000.
Life Insurance Plan. An employee must sign an application form to be eligible for this Plan and coverage will commence on the first of the month following completion of the probationary period. Should application not be made within three (3) months of this initial commencement date, the insurance company will require satisfactory evidence of insurability from the employee and his eligible dependents (i.e., spouse and unmarried, unemployed children between the ages of 14 days and twenty-five years).
Life Insurance Plan. The City shall pay one hundred percent (100%) of the premium for a term life insurance policy for each eligible employee in the amount of $50,000.
Life Insurance Plan. Benefits are based on the insurance amount in force at the time of the insured person’s death or dismemberment. The claimant must submit to the Insurer the claim and any supporting documents required by the Insurer within 10 years following the date of the insured person’s death or the accident causing the dismemberment.
Life Insurance Plan. The City pays the premiums for the following policies for each general management, supervisory, and professional employee:
Life Insurance Plan. ELIGIBILITY – 1st day of the month following completion of the initial 90 calendar days of the probationary period.
Life Insurance Plan. The Company will contribute 100% of the present premiums of the above. Non-Probationary employees shall be provided $45,000 Basic Life Insurance which is funded 100% by the Company. Non-Probationary employees shall be provided $45,000 Accidental Death and Dismemberment (AD&D) which is funded 100% by the Company.
Life Insurance Plan. Life Insurance will be procured for each employee, (excluding retired employees) equal to the amount of the gross salary received under the salary schedule for a given year, not including overtime, holiday or special pay. Dependent coverage at the amount of one thousand dollars ($1,000) per spouse and child is provided in the current plan. City agrees to pay the premium for this coverage.
Life Insurance Plan. Employees receive a life insurance benefit in the amount of fifty thousand dollars ($50,000). Dependent life insurance is available to employees at their expense.
Life Insurance Plan. A group insurance plan in the amount equal to one (1) times the nurse's annual salary will be provided for all full-time nurses who work forty-eight (48) or more hours per pay period.
