Independent Corporation. The subscriber hereby expressly acknowledges the understanding that this contract constitutes a contract solely between the subscriber and Premera Blue Cross. The subscriber further acknowledges and agrees that he or she has not entered into this contract based upon representations by any person other than us, and that no person, entity, or organization other than us shall be held accountable or liable to the subscriber for any of our obligations to the subscriber created under this contract. This provision shall not create any additional obligations whatsoever on our part other than those obligations created under other provisions of this contract. Individual Medical Plan This contract is sold and issued in Washington State as an individual medical plan. It is not issued for use as an employer-sponsored or group health plan. Premera specifically disclaims any liability for state or federal group plan requirements. This contract does not replace, affect, or supplement any state or federal requirement for worker’s compensation, employer’s liability, or similar insurance. When an employer is required by law to provide or has the option to provide worker’s compensation or similar insurance and does not provide such coverage for its employees, the benefits available under this plan will not be provided for conditions arising out of the course of employment which are or would be covered by such insurance.

This provision shall not create any additional obligations whatsoever on our part other than those obligations created under other provisions of this contract. The Provider hereby expressly acknowledges the understanding that this Agreement constitutes a contract between Provider and BCBSM, that BCBSM is an independent nonprofit corporation operating under a license with the Blue Cross Blue Shield Association (“BCBSA”). The Provider further acknowledges and agrees that he or she has not entered into this Agreement based upon representations by any person other than BCBSM, and that no person, entity, or organization other than BCBSM shall be held accountable or liable to the Provider for any of BCBSM's obligations to the Provider created under this Agreement. This paragraph shall not create any additional obligations whatsoever on the part of BCBSM other than those obligations created under other provisions of this Agreement.
Independent Corporation. PHARMACY expressly acknowledges its understanding that this Agreement constitutes a contract solely between the PHARMACY and BCBSMS, that BCBSMS is an independent corporation operating under a license with the Blue Cross and Blue Shield Association (hereinafter referred to as "the Association"), an association of independent Blue Cross and Blue Shield Plans, the Association permitting BCBSMS to use the Blue Cross and Blue Shield Service Xxxx in the State of Mississippi, and that BCBSMS is not contracting as the agent of the Association. PHARMACY further acknowledges and agrees that it has not entered into this Agreement based upon representations by any other person other than BCBSMS and that no person, entity, or organization other than BCBSMS shall be held accountable or liable to the PHARMACY for any of BCBSMS' obligations to the PHARMACY created under this Agreement. This paragraph shall not create any additional obligations whatsoever on the part of BCBSMS other than those obligations created under other provisions of this Agreement.
Independent Corporation. The Group, on behalf of itself and its participants, hereby expressly acknowledges its understanding that this Contract constitutes a contract solely between the Group and BCBSNE, which is an independent corporation operating under a license from the Blue Cross and Blue Shield Association, an association of independent Blue Cross and Blue Shield Plans, (the "Association") permitting BCBSNE to use the Blue Cross and/or Blue Shield Service Marks and that BCBSNE is not contracting as the agent of the Association. The Group, on behalf of itself and its participants, further acknowledges and agrees that it has not entered into this Contract based upon representations by any person other than BCBSNE and that no person, entity, or organization other than BCBSNE shall be held accountable or liable to the Group for any of BCBSNE's obligations created under this Contract. This paragraph shall not create any additional obligations whatsoever on the part of BCBSNE other than those obligations created under other provisions of this Contract.
Independent Corporation. The Employer on behalf of itself and its participants hereby expressly acknowledges its understanding this Contract constitutes a contract solely between the Employer and the Corporation, which is an independent corporation operating under a license from the Blue Cross® and Blue Shield® Association, an association of independent Blue Cross and Blue Shield Plans (the Association) permitting the Corporation to use the Blue Cross and/or Blue Shield service marks in the state of South Carolina, and that the Corporation is not contracting as the agent of the Association. The Employer on behalf of itself and its participants further acknowledges and agrees that it has not entered into this Contract based upon representations by any person other than the Corporation and that no person, entity or organization other than the Corporation shall be held accountable or liable to the Employer for any of the Corporation's obligations to the Employer created under this Contract. This paragraph shall not create any additional obligations whatsoever on the part of the Corporation other than those obligations created under other provisions of this Contract.
Independent Corporation. Contractholder hereby expressly acknowledge their understanding that this agreement constitutes a contract solely between contractholder and Blue Cross, which is an independent corporation operating under a license from the Blue Cross and Blue Shield Association, an association of independent Blue Cross and Blue Shield Plans, (the "Association") permitting Blue Cross to use the Blue Cross and Blue Shield Service Marks in the state of Minnesota, and that Blue Cross is not contracting as the agent of the Association. Contractholder further acknowledges and agrees that they have not entered into agreement based upon representations by any person other than Blue Cross and that no person, entity, or organization other than Blue Cross shall be accountable or liable to contractholder for any of Blue Cross’ obligations to contractholder created under this agreement. This paragraph shall not create any additional obligations whatsoever on the part of Blue Cross other than those obligations created under the provisions of this agreement. Please read the copy of the application attached to your contract. Omissions or misstatements in the application could cause an otherwise valid claim to be denied. Carefully check the application and write to us within 10 days if any information shown on the application is not correct and complete or if any information has not been included. The application is part of the contract. The contract was issued on the basis that the answers to all questions and any other material information shown on the application are correct and complete. This agreement is a legal agreement between the contractholder and Blue Cross.
Independent Corporation. RDI recognizes and acknowledges that Roman is an independent corporation to whom RDI will solely look and who is solely responsible for the obligations and liabilities of Roman recited herein, arising hereunder, or in any manner related to the transactions contemplated hereby, and RDI further recognizes and acknowledges that no other entity or entities, including any Affiliate of Roman or any individual with Roman which may supply services to or take actions on behalf of or for the benefit of Roman with respect to a Project is in any manner liable or responsible for the obligations and liabilities of Roman, whether recited in this Agreement, arising under this Agreement, or in any manner related to the transactions contemplated in this Agreement.
Independent Corporation. You, on behalf of Yourself and Your participants, hereby expressly acknowledges Your understanding that this Contract constitutes a contract solely between You and Us, an independent corporation operating under a license from the Blue Cross and Blue Shield Association, an association of independent Blue Cross and Blue Shield Plans, (the "Association") permitting Us to use the Blue Cross and/or Blue Shield Service Marks and that We are not contracting as the agent of the Association. You, on behalf of Yourself and Your participants, further acknowledge and agree that You have not entered into this Contract based upon representations by any person other than Us and that no person, entity, or organization other than Us shall be held accountable or liable to You for any of Our obligations created under this Contract. This paragraph shall not create any additional obligations whatsoever on Our part other than those obligations created under other provisions of this Contract.
Independent Corporation. Seller recognizes and acknowledges that Purchaser is an independent corporation, chartered under the laws of the State of Texas, to whom Seller will solely look and who is solely responsible for the obligations and liabilities of Purchaser recited herein, arising hereunder, or in any manner related to the transactions contemplated hereby. Seller further recognizes and acknowledges that no other entity or entities, including, but not limited to, (i) Purchaser's parent corporation, Club Resorts Holding, Inc.; the partner, Club Corporation International; or its affiliate, Club Corporation of America, (ii) any individual, or (iii) any corporation affiliated with Purchaser which may provide services to, provide loans and funds to, negotiate for, provide personnel to, make representations on behalf of, and from time to time take actions on behalf of or for the benefit of Purchaser by direct dealings with Seller or those acting for it, is in any manner liable or responsible for the obligations and liabilities of Purchaser, whether recited herein, arising hereunder, or in any manner related to the transactions contemplated hereby.
