Formal Level (Refer to Appendix C. Level 1: In the event the aggrieved person is not satisfied with the decision at the informal level, they shall appeal the decision on the appropriate form within ten (10) days of the conference stating the alleged violation of the contract, the actions that were taken, and the remedy sought. The supervisor shall attach clear, concise responses to the grievance statement and submit the decision to the unit member and to the exclusive representative within ten days after receiving the written grievance. If the supervisor does not respond within the time limit, the grievant may appeal to the next level. Within the above time limit either party may request a personal conference with the other party. Level 2: In the event the grievant is not satisfied with the decision at Level 1, the grievant may appeal the decision on the appropriate form to the Assistant Superintendent of Human Resources, or designee, within ten (10) days following the Level 1 decision. This statement shall include a copy of the original grievance, the decision rendered and a clear, concise statement of the reasons for the appeal. The Assistant Superintendent of Human Resources shall communicate his/her decision to the unit member and to the exclusive representative within ten (10) days after receiving the appeal. Either the grievant or the Assistant Superintendent of Human Resources may request a personal conference within the above time limit. If the Assistant Superintendent of Human Resources does not respond within the time limit, the grievant may appeal to the next level.   • Salary Schedule Progression Employees shall progress from step to step in salary grade on the basis of satisfactory job performance based upon established standards of performance. Seasonal employee's initial anniversary date shall be established after being in pay status for two thousand eighty (2,080) hours. Such date shall then be used for annual performance evaluation and step progression consideration. When an employee's anniversary date falls on any day from the first day of a pay week through Wednesday of the pay week, the employee's merit increase shall be effective as of the first day of the pay week within which the anniversary date falls. Otherwise, the merit increase shall be effective on the first day of the next pay week. Grievances arising from the denial of merit increases shall not be arbitrable under this Agreement but shall be processed as follows:

  • APPENDIX F Appendix F, HBITS Processes and Forms, attached hereto, is hereby expressly made a part of this Contract as fully as if set forth at length herein. OGS reserves the right to make changes to these processes and forms without seeking the prior written approval of the Department of Law or OSC, however, OGS agrees that any such changes shall comply with the terms and conditions of this Contract and not be material or substantive in nature.   • Attachment A Equity Funds This document is an attachment to the Participant Agreement with respect to the procedures to be used by (i) the Distributor and the Transfer Agent in processing an order for the creation of Shares, (ii) the Distributor and the Transfer Agent in processing a request for the redemption of Shares and (iii) the Participant and the Transfer Agent in delivering or arranging for the delivery of requisite cash payments, Portfolio Deposits or Shares, as the case may be, in connection with the submission of orders for creation or requests for redemption. The Participant is first required to have signed the Participant Agreement. Upon acceptance of the Participant Agreement by the Distributor and the Transfer Agent, the Transfer Agent will assign a PIN Number to each Authorized Person authorized to act for the Participant.
