Evidences of Insurance. Prior to the commencement of construction activities under this Agreement, the Developer will require their Contractor to file with the District a certificate of insurance (Xxxxx Form 25-S or equivalent) signed by the insurer’s representative. Such evidence shall include an original copy of the additional insured endorsement signed by the insurer's representative. Such evidence shall also include confirmation that coverage includes or has been modified to include Required Provisions 1-5. The Developer will require their Contractor, upon demand of the District, to deliver to the District such policy or policies of insurance and the receipts for payment of premiums thereon. Responsibility for Work – Until the completion and final acceptance by the City of Sun Prairie of all the work under and implied by this agreement, the work shall be under the Contractor’s responsibility care and control. The Contractor shall rebuild, repair, restore and make good all injuries, damages, re-erections, and repairs occasioned or rendered necessary by causes of any nature whatsoever. Sub-Contractors – In the event that the Contractor employs other contractors (sub-contractors) as part of the work covered by this agreement, it shall be the Contractor’s responsibility to require and confirm that each sub-contractor meets the minimum insurance requirements specified above.
Evidences of Insurance. Prior to execution of the Agreement, the Consultant shall file with the Agency a certificate of insurance (Xxxxx Form 25 or equivalent) signed by the insurer’s representative evidencing the coverage required by this Agreement. Such evidence shall include (1) attached additional insured endorsements with primary & non-contributory wording, (2) Workers’ Compensation waiver of subrogation. The Agency reserves the right to obtain complete, certified copies of all required insurance policies, at any time. Consultant shall maintain the Insurance required by this Agreement throughout the term of the Agreement and for a period of not less than 5 years following the termination of completion of this Agreement. Consultant further waives all rights of subrogation under this Agreement. Failure to continually satisfy the Insurance requirements is a material breach of contract. The Consultant shall, upon demand of the Agency, deliver to the Agency such policy or policies of insurance and the receipts for payment of premiums thereon.

Evidences of Insurance. Upon execution of this Agreement, ONTECH will, if requested by CLIENT, provide CLIENT with a certificate of insurance confirming the existence of the above described coverages.
Evidences of Insurance. Prior to execution of this Agreement, CONSULTANT shall furnish DISTRICT with original certificates and amendatory endorsements, or copies of the applicable insurance language, effecting coverage required by this Agreement. All certificates and endorsements are to be received and approved by DISTRICT before services commence. However, failure to obtain the required documents prior the services beginning shall not waive CONSULTANT’S obligation to provide them. CONSULTANT shall, upon demand of DISTRICT at any time, deliver to DISTRICT complete, certified copies or all required insurance policies, including endorsements, required by this Agreement.
Evidences of Insurance a) Prior to commencement of the Term, the Service Provider shall provide evidence of each policy of insurance required to be taken out by the Service Provider hereunder in the form of a detailed certificate of insurance and a copy of the insurance clauses so provided in such policies or if requested by the City, certified copies of the policies signed by the insurers. The certificate of insurance shall identify the Agreement title, number, policy holder, and scope of work.

Prior to execution of the agreement, and as a condition precedent to execution of the Contract by SSWA, Designer shall file with SSWA a certificate of insurance (Xxxxx Form 25-S or equivalent) signed by the insurer’s representative evidencing the coverage required by this agreement. Such evidence shall include an additional insured endorsement signed by the insurer's representative and evidence of waiver of rights of subrogation against SSWA (if builder’s risk insurance is applicable). Such evidence shall also include confirmation that coverage includes or has been modified to include and conform to the requirements of subsection 2.E.4, above. Designer shall also deliver signed copies of the policies and all endorsements, and the receipts for payment of premiums thereon. Evidence of insurance submitted by Designer shall be rejected as non- responsive if (1) any certificate of insurance does not reflect the required limits, endorsements, amendments and other requirements stated above, or (2) the entire policy with amendments is not submitted to SSWA.
