CHANGE IN PERSONAL STATUS. It is very important that you keep your personnel records up to date. Please inform the Human Resources Dept if there is ever any change in your personal status. This includes such changes as your: Legal Name Address Phone Number(s) Marital Status Withholding Exemptions Number of Dependents Beneficiary Also inform the Human Resources Department if you have completed any education courses or have obtained a degree. It is your responsibility to provide the Human Resources Department with this information. These records provide the Company with the current data so that we can communicate with you effectively. Communications sent to your last known address by registered mail shall fulfill the Company's obligation when attempts to contact you fail.
