WARNING OF RISK. The Volunteer Activities often challenge and engage the physical, mental and/or emotional resources of each volunteer. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when acting as a volunteer for the Real Life Center. All hazards and dangers cannot be foreseen. Certain risks, dangers and injuries may exist due to slips and falls, poor skill level or conditioning, carelessness, horseplay, premises defects, inadequate or defective equipment, inadequate supervision, instruction and other risks inherent to the Volunteer Activities. In this regard, it is impossible for the Real Life Center to guarantee absolute safety. EMERGENCY HEALTHCARE AUTHORIZATION In the event of your injury or illness, whether real or suspected, during your volunteer service with RLC, you authorize and give permission to take you or arrange for emergency transportation to a doctor or hospital for medical diagnosis or treatment, including but not limited to emergency surgery or medication, and you assume the responsibility of all related fees and expenses arising there from.

Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury. All hazards and dangers cannot be foreseen. Certain risks include, but not limited to, collisions between players and stationary objects, inability to stop one’s momentum and encountering off court dangers/hazards, unnecessary roughness (elbowing, hip checks, undercutting other players in the air, tripping and shoving), slip and falls, attempting a maneuver beyond the player’s skill level (i.e. attempting a dunk), poor officiating, improper personal protective equipment, slippery floors, inadequate or unsafe playing conditions, failure in supervision, unsportsmanlike conduct, dangerous/defective court conditions, and all other circumstances inherent to sport of basketball. In this regard, it is impossible for the Decatur Park District to guarantee absolute safety.

Understandably, not all hazards and dangers can be foreseen, including but not limited to overexertion, dehydration, slip and fall, collisions with other participants, the effects of weather, lack of good physical conditioning, and all other circumstances inherent to the sport of mud volleyball. WARNING OF RISK. Aerobic and other Fitness Center activities such as passive/resistive weight training, use of stair machines, jogging, free weights, and other training devices are intended to challenge and engage the physical, mental, and emotional resources of the participant. Depending on the particular activity, certain risks, dangers, and injuries due to overexertion, improper technique, ignoring safety precautions, failing to follow instructions, slips and falls, unfamiliarity with the equipment and/or exercise, equipment failure, failure in supervision/instruction, premises defects, and other risks inherent to the particular activity exist. Depending upon a person’s physical condition, age, and/or skill level, Fitness Center activities can involve a substantial risk of the following types of injuries. This list is by no means complete but includes some of the more common ones: (i) heart attack, stroke and circulatory problems, (ii) bone and joint injuries, (iii) back and neck injuries, (iv) shin splints, (v) muscle strain and other muscle injuries, and (vi) foot problems.
