User Logins. Medical Practice’s employees, representatives, consultants, contractors or agents, including Physicians and Physician Extenders, will become Users of the Software by logging into the ModMed Site indicated by Modernizing Medicine and using the login information provided by Modernizing Medicine or, with the consent of Modernizing Medicine, by the practice’s administrator. Without limiting any terms of this Agreement, after the issuance of logins to the number of Physician Users and Physician Extender Users specified as being authorized to use the Software on Schedule “A” to the License Agreement, Modernizing Medicine may make the issuance of any additional logins to Physician Users, Physician Extender Users, Resident Users and Fellow Users (e.g., upon the substitution of one Physician User for another Physician User) subject to such conditions as Modernizing Medicine may determine, including, without limitation, the payment of activation, training and other fees by Medical Practice with respect to such new Users.

User Logins. Customer agrees that the log-in access codes, user names, and/or passwords (“Logins”) provided to Customer and to other Users are personal to, and for use only by, the Users. Customer shall ensure that all such Logins are kept confidential by the applicable Users. Customer will use commercially reasonable efforts to ensure that no unauthorized person accesses the Services through Customer’s systems and interfaces. Customer is responsible and liable for all activities conducted through the Logins.

Initially, the Software shall provide access for the number of FTE Physician Users and FTE Physician Extender Users specified on Schedule “A” of the License Agreement. Part-time Physician Users and part-time Physician Extender Users will be grouped together to create a full-time or half-time Physician User or Physician Extender User, as the case may be. The determination of the number of full and half-time Physician Users and Physician Extender Users shall be made by Modernizing Medicine in its discretion. Additionally, Med Spa may have access to certain additional services, software and technology (a “Subscription”), provided that Modernizing Medicine may require Med Spa to agree to modified or additional terms in order to access such Subscription.

User Logins. Contact information for organisation survey and engagement managers will be held and used by the Cabinet Office while that individual performs this role. When the Cabinet Office is notified in writing that that individual no longer performs this role, then their contact information will be deleted. Email addresses for organisation survey and engagement managers to receive access to the survey building platform and dashboards will be stored and used by Qualtrics LLP (and their implementation partner SplitPin) while they are under contract with Cabinet Office. Individuals will be removed from the list when Cabinet Office are informed in writing that they have moved on from their role or should no longer have access. Email addresses for staff in organisations to receive access to survey results dashboards will be stored and used by Qualtrics LLP (and their implementation partner SplitPin) while they are under contract with Cabinet Office. Organisation survey and engagement managers will manage this list and individuals will be removed from the list whenever the survey or engagement manager, or Cabinet Office, are informed in writing that they should no longer have access.
