Service Logs. TES Personnel shall maintain auditable logs of Services provided. Such logs will be made available to Client, upon request, within five (5) business days. For purposes of this Agreement, the term “business days” shall not include Saturdays, Sundays or holidays.
Service Logs. The Contractor must provide and update service logs in respect of all equipment maintained by the Contractor. The Parties acknowledge that there is no additional cost to the Health Service for the Contractor providing and updating the service logs.

Service Logs. The assigned staff will provide a service log of services provided on behalf of the Client. These service logs will detail the services provided. Parent/Guardian is responsible for keeping track of service hours, dates, times of sessions, etc., but Patterns Behavioral Services, Inc., shall provide Parent/Guardian with a service log to list and summarize these activities. These service logs are required by Patterns and/or the funding source, and are submitted to funding sources to account for the services rendered on behalf of the Client.
