Patient Information Notice. This pharmacy provides an NHS pharmacist-led independent prescribing for acute conditions service. The pharmacist is a trained prescriber and can offer advice, support, and treatment to patients with a range of common illnesses. In order to provide this service the pharmacist will need to access your GP held medical records. This will allow the pharmacist to read your medical history, including details of your recent prescriptions and any drug allergies that you may have. The pharmacist will access your GP records securely using a system approved by NHS Wales. Although the pharmacist will be able to read your GP records, no information will be downloaded or retained by the pharmacy. With the exception of prescriptions which may be provided, no information will be printed. If you consent, the pharmacist will add details of your consultation onto your GP record and your doctor will be able to see this information. The pharmacist will protect your confidential information and will not share your personal details with anyone else. The pharmacy may share anonymous information with the local health board to help ensure that this service is being provided as agreed. If you do not understand this information or have any questions, please ask the pharmacist now. When you are ready, please choose one of the statements below:  I consent to the pharmacist accessing, and if necessary adding information to, my GP held medical records. I understand the information given to me about the pharmacist-led independent prescribing for acute conditions service.  I do not consent to the pharmacist accessing my GP held medical records. I understand that although the pharmacist prescriber may still be able to provide the service, that they will be required to make clinical judgements based only on the information that I have provided and a record of the consultation will be sent to my GP by email to be added to my records. Where you have chosen to consent to access of your records, this will be documented as part of the consultation notes in your GP record.
