Medical, Dental, and Vision InsuranceHealth Plans and Flexible Benefits Plan. Premium Contributions - The City shall make contributions toward the payment of premiums under the health plans currently maintained by the City for POA represented employees and any eligible dependents. For employees enrolled in a CalPERS plan, the City shall pay the required statutory minimum on behalf of each employee. The minimum contribution will apply only toward the medical insurance premium for a CalPERS Health plan. If an employee chooses not to enroll in a CalPERS Health plan, the minimum contribution cannot be used for any other purpose. The amount of the monthly Simi Flex will be increased, effective January 1, 2015, if necessary, by an amount equal to eighty percent (80%) of any premium increase in the PORAC PPO Medical Insurance Plan above the then-existing premium ($1,507.00 per month). If CalPERS changes any of the medical insurance plans by either adding to or deleting the plan options described above, employees will be limited to those plan options offered by CalPERS. Effective January 1, 2014 the required statutory minimum contribution required under PEMHCA, eligible regular employees shall be provided with a flexible benefit package (Simi Flex dollars, described in the matrix below), which may be used by the employee to offset the cost of medical, dental, or vision insurance, or other optional benefits. The Simi Flex amount will vary depending upon the medical plan and coverage level selected. It is agreed that the monthly Simi Flex will not exceed $1607.00 per month, per employee, including the PEHMCA minimum, with a maximum cash amount of $100 per month, per employee, commencing January 1, 2014. If an employee chooses not to enroll in a health plan, the employee will continue to receive Simi Flex in the amount of $100 per month.   • Medical, Dental and Vision Insurance a. Effective July 1, 2002, medical benefits shall be offered through CalPERS Health Plans.

  • Dental and Vision Insurance The Agency shall continue dental and vision plans that permit dependent coverage. The Agency shall continue to pay the premium for dental and vision coverage for the employee only. Dependent coverage shall be paid by the employee.

  • Health and Hospitalization Insurance Single Coverage: The School District shall contribute a sum not to exceed $284.00 per month toward the premium for individual coverage for each full-time employee employed by the School District who qualifies for and is enrolled in single coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.

  • MEDICAL AND HOSPITAL INSURANCE 14.1 Current practices will prevail for the duration of this Agreement, except that any changes in medical or hospital insurance plans, including the premium payable by employees, applicable to the majority of those employed in the Public Service for whom the Treasury Board is the employer, will during the life of this Agreement be applicable to the employees under this Agreement.
