Leave Bank Review Committee. All employees wishing to use the Leave Bank shall submit a “Leave Bank Request for Withdrawal Form.” This form shall be submitted to the Human Resources Office. The request shall state the maximum number of days requested by the employee. A Leave Bank Review Committee shall be established for all employees. The Leave Bank Review Committee shall consider the request of the employee. The Committee may request that the employee provide it with written verification of the catastrophic illness. Such verification must be prepared in writing by a licensed physician of the State of California. The Leave Bank Review Committee shall consist of two (2) standing Committee employees selected by the Association. The Assistant Superintendent of Human Resources or designee shall also be a classified employee of the Leave Bank Review Committee. Approval of any request shall require a majority affirmative vote of the Committee. The decision of the Committee shall be final and binding.

A general representative of the Union may be present at any meeting of the Committee. The purpose of the Committee is to deal with complaints or grievances as set out in this Collective Agreement. The Union shall keep the Hospital notified in writing of the names of the members of the Grievance Committee appointed or selected under this Article as well as the effective date of their respective appointments. A Committee member shall suffer no loss of earnings for time spent during their regular scheduled working hours in attending grievance meetings with the Hospital up to, but not including arbitration. The number of employees on the Grievance Committee shall be determined locally.

  • Nurse Representatives & Grievance Committee (a) The Hospital agrees to recognize Association representatives to be elected or appointed from amongst nurses in the bargaining unit for the purpose of dealing with Association business as provided in this Collective Agreement. The number of representatives and the areas which they represent are set out in the Appendix of Local Provisions.

  • Staffing Committee A. Responsibilities. The Nurse Staffing Committee (“NSC”) shall be responsible for determining the Institute’s staffing plan and resolving complaints raised by nurses regarding the implementation of the plan as well as those activities required of it under RCW 70.41, et seq and its successors.

  • Selection Committee A. Each building site will appoint a selection committee for the TLS. The committee shall be comprised of equal numbers of teachers and administrators and at least one teacher will be appointed by the Des Moines Education Association.

  • Training Committee The parties to this Agreement may form a Training Committee. The Training Committee will be constituted by equal numbers of Employer nominees and ETU employee representatives and have a charter which clearly states its role and responsibilities. It shall monitor the clauses of this Agreement which relate to training and ensure all employees have equal access to training.

  • Benefits Advisory Committee The Board agrees to establish a Benefits Advisory Committee to provide stakeholder input into maintaining quality and affordable benefits. The focus of this committee shall be to:

  • Education Committee (a) The Employer will establish an Education Committee for all employees in the facility, which shall include at least one representative from ONA members.
