Compliance Coordinators. The District shall designate at least one employee to coordinate its efforts to comply with and carry out its responsibilities under Title II of ADA, including any investigation of any complaint communicated to it alleging its noncompliance or alleging any actions that would be prohibited under ADA. The District shall make available to all interested individuals the name, office address, and telephone number of the employee(s) so designated and shall adopt and publish procedures for the prompt and equitable resolution of complaints alleging any action that would be prohibited under the ADA. The school system receives federal financial assistance and must comply with the above requirements. Additionally, the School Committee is of the general view that:

Compliance Coordinators. A. HDOE agrees that the responsibilities of its Title IX and its Section 504/Title II coordinator(s) are essential to its commitment to compliance with Title IX, Section 504 and Title II. HDOE will designate one or more individuals to coordinate its efforts to comply with and carry out its responsibilities under the regulations enforced by OCR; including the prompt and equitable resolution of complaints of discrimination and harassment filed by students, parents, employees, and other third parties. HDOE will require its Title IX and Section 504/Title II coordinator(s) to have the following responsibilities and mandatory training requirements:

The District will revise all relevant policies, procedures and publications to include the name/title, office address, telephone number and electronic mail (email) address of the person(s) designated to coordinate its efforts to comply with the regulations enforced by OCR. The District will ensure that contact information for the coordinator(s) is consistent and complete in all such materials. REPORTING REQUIREMENT: By February 12, 2015, the District will inform OCR of the identity and qualifications of the individual(s) designated to coordinate its efforts to comply with and carry out its responsibilities under the regulations enforced by OCR; and the specific responsibilities of the coordinator(s), including the issue areas and geographical areas for which the coordinator(s) will be responsible. Additionally, the District will provide documentation to OCR demonstrating that the information listed above about the coordinator(s) has been provided consistent with the Action Item above; such as a copy of revised materials and publications containing this information, and/or a link to the on-line version of the materials and publications.   • Service Coordinators Each Party has designated an employee or title as the key contact for the day-to-day implementation or monitoring of each Service as specified in the applicable Transition Service Schedule (each, a “Service Coordinator”). The Parties shall direct communications relating to specific Services to the applicable Service Coordinators. The Service Coordinators shall report to the Transition Committee from time to time, as directed by the members of the Transition Committee designated by the applicable Party.

  • Project Coordinator 3. Within 14 days of the effective date of this Consent Agreement, DTSC and Respondent shall each designate a Project Coordinator and shall notify each other in writing of the Project Coordinator selected. Each Project Coordinator shall be responsible for overseeing the implementation of this Consent Agreement and for designating a person to act in his/her absence. All communications between Respondent and DTSC, and all documents, report approvals, and other correspondence concerning the activities performed pursuant to this Consent Agreement shall be directed through the Project Coordinators. Each party may change its Project Coordinator with at least seven days prior written notice. WORK TO BE PERFORMED

  • Care Coordination MCOs must ensure care coordination is provided to Members with a substance use disorder. MCOs must work with providers, facilities, and Members to coordinate care for Members with a substance use disorder and to ensure Members have access to the full continuum of Covered Services (including without limitation assessment, detoxification, residential treatment, outpatient services, and medication therapy) as Medically Necessary and appropriate. MCOs must also coordinate services with the DSHS, DFPS, and their designees for Members requiring Non-Capitated Services. Non-Capitated Services includes, without limitation, services that are not available for coverage under the Contract, State Plan or Waiver that are available under the Federal Substance Abuse and Prevention and Treatment block grant when provided by a DSHS-funded provider or covered by the DFPS under direct contract with a treatment provider. MCOs must work with DSHS, DFPS, and providers to ensure payment for Covered Services is available to Out-of-Network Providers who also provide related Non-capitated Services when the Covered Services are not available through Network Providers.

  • Alliance Manager Each Party shall appoint a person(s) who shall oversee contact between the Parties for all matters between meetings of each Joint Committee and shall have such other responsibilities as the Parties may agree in writing after the Effective Date (each, an “Alliance Manager”). Each Party may replace its Alliance Manager at any time by notice in writing to the other Party.

  • Alliance Managers In addition to the foregoing governance provisions, each of the Parties shall appoint a single individual to serve as that Party’s alliance manager (“Alliance Manager”). The role of each Alliance Manager will be to participate and otherwise facilitate the relationship between the Parties as established by this Agreement.
