Clinical Service Provision. Provide occupational therapy services to inpatients, outpatient/same day patients which may include clients of other MH&ACCQL facilities as required; • Provide accurate client assessment, treatment programs and progress, within the client’s medical record to facilitate communication with other team members documenting each client’s clinical pathway daily; • Prepare written reports to referring agencies and other professionals as required; • Ensure that documentation complies with hospital, legal, professional standards; • Provide assessment, diagnosis and treatment of inpatients and outpatients/same day patients; • Provide counselling and education to patients and families in relation to their individual therapy programs; • Demonstrate a positive approach towards the care of clients and their significant other; • Liaise with other Health Care Professionals, clients’ families and external agencies as required to achieve optimum outcomes in regards to care; • Participate in team decision making by attending weekly case conferences to report on clients’ progress; • Communicate effectively with clients, care givers and professional colleagues; • Assess and prescribe medical equipment and assistive devices; and • Assess and prescribe fabrication of splinting devices.
Clinical Service Provision. The Aurora/Denburn Medical Practice grouping currently provides General Medical Services and works closely with aligned community health and social care services to provide integrated care for their practice population of 16,337 across the three locations, Denburn Health Centre, Northfield Clinic and Mastrick Clinic. From April 2017, a Triage Model of Care, initially developed within the Denburn Medical Practice, was in place serving all three locations. There are arrangements in place to help manage clinical demand between the three sites using remote telephonic working but the geographical separation, poor state of repair and functional limitations on building design, significantly impede the practice’s plans to improve range, quality and access to services, through the introduction of innovative models of service delivery and more efficient working practices. These restrictions also inhibit the ability to involve other healthcare professional disciplines in the model of care.
