Bargaining Unit Employees and Dependents. The Company will continue to pay the full cost of the Ontario Hospital Insurance Plan. • The Company will provide Vision Care with a $375.00 per family maximum for the term of this Agreement. The company will also provide two (2) eye examinations per family per term of the agreement. This program excludes contact lenses. • The Company will provide a drug plan under which the first $20.00 of expense per annum will be paid by a married employee and the first $10.00 of expense per annum will be paid by a single employee. There will be no co-insurance feature in the drug plan. • The drug plan will use the Drug Card System with a stipulation of drug card formulary having generic drugs as first option. When a generic equivalent is not available, the doctor specified drug will be first option. • Except as may be altered by this Schedule, the Company will continue the present Medicare Supplement Plan. • The Company will continue the dental plan which was introduced August 1, 1979. There will be a deductible sum of $25.00 per family per year with a maximum benefit of $1,250 per person per year. The specifications of the dental plan shall be the same as those attached to this Schedule. • Effective on the dates shown below, the level of benefits under the Dental Plan will be amended by adopting the Ontario Dental Association’s schedule of fees which has been published for the corresponding year: Effective Date Year August 17, 2008 2007 August 17, 2009 2008 August 17, 2010 2009 A.3 The Company will pay the full cost of the foregoing group insurance benefits and the Pension Plan will be non-contributory.

  • Shift Employees Employees who work rotating shift patterns or those who work qualifying shifts shall be entitled, on completion of 12 months employment on shift work, to up to an additional 5 days annual leave, based on the number of qualifying shifts worked. The entitlement will be calculated on the annual leave anniversary date. Qualifying shifts are defined as a shift which involves at least 2 hours work performed outside the hours of 8.00am to 5.00pm, excluding overtime.   • Bargaining Unit Seniority The length of continuous service in a position or succession of positions within Bargaining Unit Two (2), beginning with the last date of hire or transfer into the Bargaining Unit, as defined by seniority credits.

  • All Employees The Company shall not include the shift differential in any employee’s wage rate for the calculation of overtime.

  • Project Employment 1. The Employer may appoint employees into project positions for which employment is contingent upon state, federal, local, grant, or other special funding of specific and of time-limited duration. The Employer will notify the employees, in writing, of the expected ending date of the project employment.

  • Bargaining Unit Roster Upon the signing of this Agreement and monthly thereafter, the Employer shall supply to the Union via a secured method an alphabetical list of all employees covered by this Agreement. The list shall include the name, address, employee identification number, date of hire, rehire date (if applicable), shift, FTE, job classification, department cost center number, unit, hourly rate of pay and monthly gross earnings. Each month, the Employer will provide a list of new hires and addresses, and a list of all employees who have terminated during the month via a secured method. The new hire and termination lists shall include the same data as the monthly employee roster except for monthly gross earnings. The termination list shall include the termination date. Within ninety (90) days of ratification, Swedish Medical Center and SEIU 1199NW will convene a work group including HRIS expert to explore a method for the Employer to provide a list of all employment changes for bargaining unit employees, via a secure site.
