Services Program. The Trauma Center agrees readiness services costs covered by this agreement are for the period 1) Maintain “Trauma Center” designation by the Department of Public Health (DPH) throughout the duration of this Agreement. 2) Ensure that at least 25 percent of “Readiness Services Program” funds will be or has been paid to eligible physicians providing trauma related services to trauma patients receiving such services at Trauma Center’s facility during covered period. 3) Provide to Commission a final report at end of agreement period documenting the use of Readiness Services Program funding provided through Agreement. For FY 2021, the Commission has determined sixty percent (60%) of Readiness Services Program funding available to Level III Trauma Centers will be linked to the Performance Based Program Services (PBP). Total Readiness Services Program funding awarded to Trauma Center will include the PBP funding determined by the satisfaction of PBP criteria. The PBP Scorecard included in the contract must be submitted to the Commission office on or before 1 April 2021. Trauma Centers will be notified in May 2021 of compliance to PBP criteria, and total amount of Readiness Services Program funding to be awarded for FY 2021. Performance Based Program Service Criteria for Level III Trauma Centers are: 1) Participation in Trauma Medical Directors (TMD) Conference Calls. Seventy-five percent (75%) call attendance by TMD or another designated physician representative is required to satisfy this criterion. (5% value) 2) Participation by trauma program manager or other designated representative in Georgia Committee for Trauma Excellence (GCTE) meetings. Seventy-five percent (75%) attendance by trauma program manager or other designated representative at GCTE meetings is required to satisfy this criterion. Meeting attendance rosters will be used to verify attendance. (5% value) 3) Attendance at the 2021 Spring Symposium, COT & TQIP Collaborative meeting at Chateau Elan by both the Trauma Medical Director (or designated physician) and the Trauma Program Manager (or designee). (5% value) 4) Participation in Trauma Administrators Group by senior executive accountable for the trauma program or designated executive that is not the Trauma Program Manager equivalent. Services Program. 3.1 The following services will be provided by Life House: facilitate out-of-school time youth recreational programming in the City's Central Hillside Neighborhood. Recreational programming may include a combination of indoor and outdoor recreation, both onsite and off-site including but not limited to City Parks, Trails, and other recreational locations to help youth enjoy safe, structured, healthy recreation. Services Program. The Parties agree that GEE, as an Educational Service Provider to the Academy, to the extent permitted by and in conformity with the Contract and applicable federal, state and local laws, rules and regulations shall provide all curriculum, technology, computers, marketing, staffing, student pupil services, facility, compliance, student assessments, extracurricular activities, management and operation, materials, equipment, supplies, supervision, compensation, benefits, payroll taxes, insurances, and the like necessary for the management, operation and delivery of the Program to the students enrolled in the Academy assigned to the Program to the extent not otherwise the obligation of the Academy with respect to funds paid to the Academy and for which such a student is eligible. GEE shall do whatever is necessary to carry out the Program for the Academy students covered by this Agreement including, but not limited to, providing the curriculum, method of pupil assessment, development of admissions policies and criteria, the school calendar, the school day schedule, age and grade range of pupils to be enrolled, and educational goals and methods to be used to monitor the compliance with the performance of targeted educational outcomes as adopted by the Academy Board and included in the Contract. As an Educational Service Provider to the Academy, GEE operates the GEE Preparatory Learning Center wherein eligible Michigan public school students are provided with learning courses that are supplemental to or in place of traditional, learning courses that may otherwise be available to such students (the “Program”). GEE shall make the Program available to Academy students pursuant to the terms of this Agreement. GEE shall ensure that the Program meets all requirements of the applicable federal, state and local laws, rules and regulations, including, but not limited to, the Michigan Merit Curriculum, the Code, the Michigan Pupil Accounting Manual and the Contract so that Academy pupils enrolled in the Academy and assigned to the program are counted in membership for purposes of the Academy’s receipt of the State School Aid per pupil foundational allowance. ▇▇▇ shall have the authority to take whatever action necessary on behalf of the Academy to ensure that the Academy receives the per pupil state foundational allowance for each pupil enrolled and assigned to the Program.
