I understand that the Program’s discovery of my failure to disclose a criminal conviction may lead to dismissal from the Program. I also understand that my Program participation may be terminated if a criminal conviction results in disqualification from my graduate Program.

I understand that the CWTAP’s discovery of my failure to disclose a conviction may lead to dismissal from the CWTAP. I understand that DCYF must review and approve the CWTAP participation in light of any criminal conviction and that the CWTAP participation may be terminated if a conviction results in disqualification from DCYF service. _____ I agree to submit to criminal background and FamLink checks during the application and admission process, at the start of each practicum, and at any time it is requested by the CWTAP. _____ I agree to maintain a valid driver license during my time in CWTAP including my employment service period. _____ I will maintain my status as a citizen or as a permanent resident of the U.S. during participation in the program, including the Employment Service Period. _____ I agree to meet the high standards of professional and ethical behavior required by the university and as outlined in the National Association of Social Workers Code of Ethics.
Prescreening and Program Provisions. Student agrees to complete and maintain satisfactory academic progress and performance in courses, training, seminars, field placements and other surveys or instruments designated by the University to satisfy the requirements within the specific time. If Student has not completed the degree within the specific time frame, and if it is determined by the University that a good faith effort to complete the academic requirements has been made, Student may request a modification of the time frame by which completion of the academic requirements must be achieved. Student has a duty to disclose any previous or subsequent conviction of any crime that disqualifies individuals from field placement or employment in a county public mental health agency, or a community-based organization under contract to a county public mental health agency, which will render Student ineligible for, or cause termination from, the Program. Student agrees, within the first semester or quarter of Program enrollment, to be pre-screened for mental health employment, to be fingerprinted and to participate in the criminal clearance process pursuant to Department of Justice or other applicable screening regulations governing mental health employment in California. Unless specifically waived by the University in writing, Xxxxxxx agrees to maintain a valid driver’s license and any insurance required by the field placement or payback employer. Student is committed to seek and obtain employment for one calendar year for each academic year for which the stipend is granted, within the specified time frame in a county public mental health agency, or a community-based organization under contract to a county public mental health agency.

  • Clinical Data and Regulatory Compliance The preclinical tests and clinical trials, and other studies (collectively, “studies”) that are described in, or the results of which are referred to in, the Registration Statement or the Prospectus were and, if still pending, are being conducted in all material respects in accordance with the protocols, procedures and controls designed and approved for such studies and with standard medical and scientific research procedures; each description of the results of such studies is accurate and complete in all material respects and fairly presents the data derived from such studies, and the Company and its subsidiaries have no knowledge of any other studies the results of which are inconsistent with, or otherwise call into question, the results described or referred to in the Registration Statement or the Prospectus; the Company and its subsidiaries have made all such filings and obtained all such approvals as may be required by the Food and Drug Administration of the U.S. Department of Health and Human Services or any committee thereof or from any other U.S. or foreign government or drug or medical device regulatory agency, or health care facility Institutional Review Board (collectively, the “Regulatory Agencies”); neither the Company nor any of its subsidiaries has received any notice of, or correspondence from, any Regulatory Agency requiring the termination, suspension or modification of any clinical trials that are described or referred to in the Registration Statement or the Prospectus; and the Company and its subsidiaries have each operated and currently are in compliance in all material respects with all applicable rules, regulations and policies of the Regulatory Agencies.

  • Training and Promotion a. The contractor will assist in locating, qualifying, and increasing the skills of minorities and women who are applicants for employment or current employees. Such efforts should be aimed at developing full journey level status employees in the type of trade or job classification involved.

  • Evaluation, Testing, and Monitoring 1. The System Agency may review, test, evaluate and monitor Grantee’s Products and services, as well as associated documentation and technical support for compliance with the Accessibility Standards. Review, testing, evaluation and monitoring may be conducted before and after the award of a contract. Testing and monitoring may include user acceptance testing. Neither the review, testing (including acceptance testing), evaluation or monitoring of any Product or service, nor the absence of review, testing, evaluation or monitoring, will result in a waiver of the State’s right to contest the Grantee’s assertion of compliance with the Accessibility Standards.

  • Research Use Reporting To assure adherence to NIH GDS Policy, the PI agrees to provide annual Progress Updates as part of the annual Project Renewal or Project Close-out processes, prior to the expiration of the one (1) year data access period. The PI who is seeking Renewal or Close-out of a project agree to complete the appropriate online forms and provide specific information such as how the data have been used, including publications or presentations that resulted from the use of the requested dataset(s), a summary of any plans for future research use (if the PI is seeking renewal), any violations of the terms of access described within this Agreement and the implemented remediation, and information on any downstream intellectual property generated from the data. The PI also may include general comments regarding suggestions for improving the data access process in general. Information provided in the progress updates helps NIH evaluate program activities and may be considered by the NIH GDS governance committees as part of NIH’s effort to provide ongoing stewardship of data sharing activities subject to the NIH GDS Policy.
