Letter of Counselling. An in-scope supervisor or a manager may give an employee a letter of counselling designed to improve the employee’s performance or conduct, which outlines performance expectations. The employee may provide a written rebuttal to the Employer’s letter of counselling within a reasonable time. Neither the letter of counselling nor the rebuttal will be placed on the employee’s Personnel File. Supervisors and employees are encouraged to share feedback on progress towards meeting the expectations identified in the counselling letter. By employee written request, counselling letters more than two years old will be cleared from the supervisor’s employee file if no further counselling letters, adverse reports or disciplinary actions have been submitted.

Letter of Counselling. The Director of Operations may give an employee a letter of counseling designed to improve the employee’s performance or conduct, which outlines performance expectations. The employee may provide a written rebuttal to the letter of counseling within a reasonable time. Neither the letter of counseling nor the rebuttal will be placed on the employee’s Personnel File.
Letter of Counselling. If the performance or behaviour problem persists or if a similar infraction occurs, the Executive Director will again meet with the employee to discuss the problems. Following this meeting, a letter of counselling will be issued to the employee outlining what occurred at the meeting. This letter becomes part of the employee’s Personnel File and a copy of that letter will be forwarded directly to the employee. The employee will be advised that failure to correct (or a repeated infraction) may result in further disciplinary action taken against the employee up to and including dismissal.

  • Certificates of Insurance/Notices The Contractor shall provide OGS with a Certificate or Certificates of Insurance, in a form satisfactory to OGS as detailed below (e.g., an XXXXX certificate), after renewal or upon request. Certificates shall reference the award number and shall name The New York State Office of General Services, Procurement Services, 38th Xxxxx, Xxxxxxx Xxxxx, Xxxxxx Xxxxx Xxxxx, Xxxxxx, Xxx Xxxx 00000 as the certificate holder. Certificates of Insurance shall:  Be in the form acceptable to OGS and in accordance with the New York State Insurance Law (e.g., an XXXXX certificate);  Disclose any deductible, self-insured retention, aggregate limit or exclusion to the policy that materially changes the coverage required by this Contract;  Refer to this Contract by award number;  Be signed by an authorized representative of the referenced insurance carriers; and  Contain the following language in the Description of Operations / Locations / Vehicles section: The People of the State of New York, the New York State Office of General Services, any entity authorized by law or regulation to use the Contract and their officers, agents, and employees are included as an additional insured on endorsement CG 20 10 11 85 (or endorsements that provide equivalent coverage, such as the combination of CG 20 10 04 13 (covering ongoing operations) and CG 20 37 04 13 (covering completed operations)), and General liability coverage is provided on the current edition of Commercial General Liability Coverage Form CG 00 01 (or a form that provides equivalent coverage). Additional insured protection afforded is on a primary and non-contributory basis. A waiver of subrogation is granted in favor of the additional insureds. Only original documents (certificates of insurance and any endorsements and other attachments) or electronic versions of the same that can be directly traced back to the insurer, agent or broker via e-mail distribution or similar means will be accepted. OGS generally requires Contractors to submit only certificates of insurance and additional insured endorsements, although OGS reserves the right to request other proof of insurance. Contractors are requested to refrain from submitting entire insurance policies, unless specifically requested by OGS. If an entire insurance policy is submitted but not requested, OGS shall not be obligated to review and shall not be chargeable with knowledge of its contents. In addition, submission of an entire insurance policy not requested by OGS does not constitute proof of compliance with the insurance requirements and does not discharge Contractors from submitting the requested insurance documentation.

  • Certificate of Insurance/Endorsements A certificate of insurance from an insurer with a Best's rating of no less than A- indicating compliance with the required coverages has been received by the Department’s Contracts Management Bureau, X.X. Xxx 000000, Xxxxxx, XX 00000. Contractor must notify the Department immediately of any material change in insurance coverage, such as changes in limits, coverages, change in status of policy, etc. The Department reserves the right to require complete copies of insurance policies at all times.

  • Certificate of Insurance Contractor must provide a Certificate of Insurance form to the City of Sparks to evidence the insurance policies and coverage required of Contractor.

  • Certificate of Conformance At Buyer’s request, a Certificate of Conformance stating the Products conform to all Order requirements shall accompany each shipment. Seller shall have available for review all sub-tier certifications for goods and processes that support the shipment.

  • Certificate of Compliance The Servicer shall deliver to the Note Issuer, the Note Trustee, the Certificate Trustee and the Rating Agencies on or before March 31 of each year, commencing March 31, 2006 to and including the March 31 succeeding the Retirement of the Notes, an Officer’s Certificate substantially in the form of Exhibit A hereto (a “Certificate of Compliance”), stating that: (i) a review of the activities of the Servicer during the twelve months ended the preceding December 31 (or, in the case of the first Certificate of Compliance to be delivered on or before March 31, 2006, the period of time from the date of this Agreement until December 31, 2005) and of its performance under this Agreement has been made under such Responsible Officer’s supervision, and (ii) to the best of such Responsible Officer’s knowledge, based on such review, the Servicer has fulfilled all of its obligations under this Agreement in all material respects throughout such twelve months (or, in the case of the Certificate of Compliance to be delivered on or before March 31, 2006, the period of time from the date of this Agreement until December 31, 2005), or, if there has been a failure to fulfill any such obligation in any material respect, specifying each such failure known to such Responsible Officer and the nature and status thereof.
