APPENDIX 4A. The Plymouth 4-Way Agreement This signed statement is drawn up to help you and your team develop an effective treatment programme. This involves you, your Pharmacy, your Key worker and your Prescribing Doctor working together in a way that is respectful of each other. To avoid any confusion later the following points are raised: • Each patient is personally responsible for collecting their own "script' at a suitable time arranged with the pharmacy. It is best to come alone. • If you miss or loose a dose, this cannot be replaced later. • Your "script' cannot be released to anyone else without authorisation from your Key worker or Prescribing Doctor. • To help locum Pharmacists please supply a photo which will be kept safe and confidentially in a locked draw at the Pharmacy. It is the Pharmacist's job to check if someone is fit to take medication. For safety reasons, your dose will be withheld if you are intoxicated. The Pharmacy will inform your Key worker / Prescriber of difficulties occurring on their premises in the interests of you receiving effective treatment. PLEASE REMEMBER that your "script" may be stopped if: • You do not collect your treatment. • You behave unacceptably. • You don't show up for Key worker or Doctor appointments. • Your team decide it is not safe to continue with it. We hope this will never happen and look forward to working in a helpful way together as many have done already. Any concerns you have about the service will be taken seriously and we welcome suggestions for improvements.
