ADDITIONAL CONTRACTOR RESPONSIBILITIES. 21 CONTRACTOR shall notify ADMINISTRATOR, in writing, within twenty-four 22 (24) hours if the position for CONTRACTOR’S physician for Child Abuse Consultation and 23 Medical Evaluation Services becomes vacant due to resignation, termination, leave of absence, or 24 other reason. 25 CONTRACTOR shall deliver culturally responsive and linguistically appropriate 26 services to meet the primary language needs of those served.

Provide ADMINISTRATOR a bi-annual detailed marketing plan for each contracted service, and revise, if necessary, as requested by ADMINISTRATOR. Actively engage the community, including local residents, faith-based groups, businesses, public and private organizations, civic groups, and others in the planning and implementation of services that promote the well-being, safety, and permanency of children, families, and communities.
ADDITIONAL CONTRACTOR RESPONSIBILITIES. On a monthly basis, CONTRACTOR shall allocate funds to provide PROGRAM PARTICIPANTS with housing to include rent, utilities, transportation, purchase of food for meals and snacks, kitchen utensils, cleaning supplies, clothing, telephone, and other necessities. Personal Needs CONTRACTOR shall: Provide initial supplies for personal care items, including, but not limited to, toothpaste, toothbrush, soap, hair care items, and hygienic supplies. CONTRACTOR shall inform PROGRAM PARTICIPANTS that subsequent purchases of personal care items are the responsibility of the PROGRAM PARTICIPANT. Personal items shall be the property of each PROGRAM PARTICIPANT and shall be retained by PROGRAM PARTICIPANT upon exiting the program. Provide initial supply of clean towels, mattress pads, pillows, sheets, and blankets. Provide a secure, separate storage area for personal items for each PROGRAM PARTICIPANT. Absence CONTRACTOR shall: Notify ADMINISTRATOR if PROGRAM PARTICIPANT has an unauthorized absence as defined in Subparagraph 2.19 of this Attachment. Consult with the CFS Liaison, ASW, or DPO if a PROGRAM PARTICIPANT’s whereabouts are unknown and reasonable suspicion exists that PROGRAM PARTICIPANT is a victim of harm or foul play, or has competency issues.

ADDITIONAL CONTRACTOR RESPONSIBILITIES. In addition to providing the services described in Paragraph of this Exhibit B, CONTRACTOR agrees to: Appear and testify at Juvenile Court hearings. Notify the assigned social worker, by telephone, within two (2) days if any of the following occurs: FAMILY fails to comply with first in-home visit. Provide the social worker with the date of the rescheduled home visit. FAMILY fails to comply with the second in-home visit. Notify the social worker if the family misses a total of three in-home visits during the service period, and discontinue services. Terminate the FAMILY within ten (10) working days, if the assigned social worker does not call to request that the FAMILY be reinstated. A FAMILY may be reinstated only once during the service period. The reinstated FAMILY will receive the next available slot for service, and will be required to sign a new release of information form and new service agreement.
