Sick Leave Bank Review Committee. All unit members wishing to use the Sick Leave Bank shall submit a “Sick Leave Bank Request for Withdrawal Form.” This form shall be submitted to the Human Resources Office. The request shall state the maximum number of days requested by the unit member. A Sick Leave Bank Review Committee shall consider the request of the unit member. The Committee may request that the unit member provide it with written verification of the catastrophic illness. Such verification must be prepared in writing by a licensed physician of the State of California. The Sick Leave Bank Review Committee shall consist of four standing Committee members selected by the Association. The Assistant Superintendent, Human Resources, shall also be a member of the Sick Leave Bank Review Committee. Approval of any request shall require a majority affirmative vote of the Committee. The decision of the Committee shall be final and binding.

  • Grievance Committee The Hospital will recognize a Grievance Committee composed of the Chief Xxxxxxx and not more than (as set out in Local Provisions Appendix) employees selected by the Union who have completed their probationary period. A general representative of the Union may be present at any meeting of the Committee. The purpose of the Committee is to deal with complaints or grievances as set out in this Collective Agreement. The Union shall keep the Hospital notified in writing of the names of the members of the Grievance Committee appointed or selected under this Article as well as the effective date of their respective appointments. A Committee member shall suffer no loss of earnings for time spent during their regular scheduled working hours in attending grievance meetings with the Hospital up to, but not including arbitration. The number of employees on the Grievance Committee shall be determined locally.
