The following performance targets have been agreed between the Parties;  The Provider to have invited 100% of claimants referred to the Provision to a face to face meeting which must be booked to take place within 5 working days of the referral to the Provision  90% of claimants referred to the 3 week support to attend and participate fully for the duration of the support period;  All claimants to receive focussed and ongoing mentoring support in carrying out the agreed action plan for 3 weeks and to also include support for the 13 weeks In Work Support where required;  The Individual Action Plan, must also detail attendance rates, skills developed, distance travelled and digital competence;  Job Outcomes – a minimum of 30% of the claimants referred and started on the provision by JCP to cease claiming any DWP qualifying benefit and move into qualifying employment within 6 weeks of completion of the provision. Job Outcomes must be genuine opportunities with a realistic possibility of providing sustained employment for the Claimant and be expected to last a minimum of 13 weeks and result in an off flow from benefits. .

The following performance targets have been agreed between the Parties;  100% of participants to have had their initial financial assessment within 5 days of referral.  100% of participants to have received the minimum number of four contacts.  100% of participants to have an agreed Individual Personal Finance Action Plan.  Minimum of 80% of participants completing the agreed programme and all their activities within their Personal Finance Action Plan.  All participants who complete the programme to have a completed Exit Plan.  All participants who complete the programme to have their confidence and knowledge of budgeting considerably improved. This will be evaluated by comparing the initial assessments with the Personal Finance Action Plans and Exit Plans.  Minimum of 40% of participants who commence the programme and have a Personal Finance Action Plan completed to start employment (of at least 16hrs per week), within 13 weeks of leaving the programme.  Minimum of 50% of participants who commence the programme and have a Personal Finance Action Plan completed to have commenced other suitable contracted/non-contracted provision as Performance Target referred/signposted by the successful bidder or DWP Adviser.

The following performance targets have been agreed between the Parties; Performance Targets • All Initial Assessments must be undertaken within 5 days of referral from JCP (see paragraph 4.4 of Specification); • Provider must furnish JCP with a copy of each ‘signed off’ Initial Assessment Report within 5 days of the Assessment being undertaken (see paragraph 3.6 of Specification); • All claimants must be offered a suitable community work place tasters (as detailed in paragraph 3.10 of Specification); • The Provider will be required to supply JCP timeously with regular monthly performance data (e.g. starts / completers / leavers / job outcomes data). Full details of reporting requirements will be agreed with JCP following award of contract; • The Provider must furnish JCP with a copy of each Individual Final Assessment Report within 5 days of the course ending (see paragraph 3.11 of Specification);

The following performance targets have been agreed between the Parties; 30% of Claimants, who complete the 6 week Opportunity Health Support course to achieve a job outcome;  100% of Claimants who started and completed the Opportunity Health Support Provision to have their IAP reviewed weekly, including a final review;  90% of Claimants who start on the provision and reach the 3 week stage, to participate fully and complete the 6 week Opportunity Health Support provision; 
