Initial Assessments. All clients who are admitted to the SRF will undergo the following assessments: o Physical examination and physician’s report completed before admission o Activities assessment by the SRF Program Director during admission o Initial evaluation/care and medication plan completed by a licensed nurse during admission o Initial mental health evaluation/assessment completed and signed by a licensed mental health professional during the time of admission. • Within 72 hours of admission, the Program Director in collaboration with the client, client’s representative, family (where applicable), licensed nurse, and licensed mental health professional will develop an Individual Treatment Plan for the client. This Plan will be updated every 60 days or as needed. • Client Orientation – Prior to admission, the Program Director will discuss the range of services the SRF provides with the client and client’s representative, and how the services of the SRF may benefit the client. Upon admission, the Program Director and Nurse will review all client’s specific plans, treatment and rehabilitation protocols, and process with the client, including the process of developing an Individual Treatment Plan. The client will be assigned a bed, a schedule for meal times, activities’ schedules, and housekeeping rules. A direct care staff will be assigned to the client to tour the facility and provide the client with fire and safety rules, and evacuation plans.

Examples of Initial Assessments in a sentence

  • The Department will pay for Initial Assessments, Continued Stay Reviews, and Waitlist Assessments from data pulled from the Department prescribed system on the eighteenth (18) day of the month for assessments from the previous month.

  • Conduct Triennial and Initial Assessments for the purpose of identifying special education eligibility.

Related to Initial Assessments

  • Initial assessment means an assessment conducted prior to or at admission to determine whether the individual meets the service's admission criteria; what the individual's immediate service, health, and safety needs are; and whether the provider has the capability and staffing to provide the needed services.

  • Special Assessments means any of the following, except as otherwise provided in subsection (6):

  • Special assessment means an assessment against real property calculated on a benefit or ad valorem basis for curb and gutter, sidewalk, sewer, water, or street paving; a drain; a connection fee or similar charge for a sewer or water system; or the land contract on a parcel of property acquired under the circumstances set forth in section 3(2). Special assessment does not include charges for current service.

  • Initial assessed value means the assessed value of all the taxable property within the boundaries of a zone at the time the tax increment financing plan is approved, as shown by the most recent equalized assessment roll of the city, village, or township at the time an agreement is approved under this section. Property exempt from taxation at the time of the determination of the initial assessed value shall be included as zero. For the purpose of determining initial assessed value, property for which a specific local tax is paid in lieu of a property tax shall not be considered to be property that is exempt from taxation.

  • Phase I assessment as described in, and meeting the criteria of, (i) Chapter 5 of the FNMA Multifamily Guide or any successor provisions covering the same subject matter in the case of a Specially Serviced Mortgage Loan as to which the related Mortgaged Property is multifamily property or (ii) the American Society for Testing and Materials in the case of Specially Serviced Mortgage Loan as to which the related Mortgaged Property is not multifamily property.
