Initial Assessments definition

Initial Assessments. All clients who are admitted to the SRF will undergo the following assessments: o Physical examination and physician’s report completed before admission o Activities assessment by the SRF Program Director during admission o Initial evaluation/care and medication plan completed by a licensed nurse during admission o Initial mental health evaluation/assessment completed and signed by a licensed mental health professional during the time of admission. • Within 72 hours of admission, the Program Director in collaboration with the client, client’s representative, family (where applicable), licensed nurse, and licensed mental health professional will develop an Individual Treatment Plan for the client. This Plan will be updated every 60 days or as needed. • Client OrientationPrior to admission, the Program Director will discuss the range of services the SRF provides with the client and client’s representative, and how the services of the SRF may benefit the client. Upon admission, the Program Director and Nurse will review all client’s specific plans, treatment and rehabilitation protocols, and process with the client, including the process of developing an Individual Treatment Plan. The client will be assigned a bed, a schedule for meal times, activities’ schedules, and housekeeping rules. A direct care staff will be assigned to the client to tour the facility and provide the client with fire and safety rules, and evacuation plans.
Initial Assessments means the assessments levied on Assessed Property within Improvement Area #1 pursuant to the Initial Assessment Order.

Examples of Initial Assessments in a sentence

  • The Department will pay for Initial Assessments, Continued Stay Reviews, and Waitlist Assessments from data pulled from the Department prescribed system on the eighteenth (18) day of the month for assessments from the previous month.

  • Conduct Triennial and Initial Assessments for the purpose of identifying special education eligibility.

  • Initial Assessments Forms will be completed by all customers; these will be utilized to identify service needs and in making referrals to appropriate on-site and off-site Partners and agencies.

  • Documentation of Initial Assessments and Requests for Auxiliary Aids or Services or Language Assistance.

  • Initial Assessments are billed at $175 per hour plus travel time, billed at 10 minute increments.
