Offer Checklist Розділ 2. Необхідні документи To assist offerors in preparation of the quote, the following checklist summarizes the documentation to include an offer in response to this RFQ:  Cover letter, signed by an authorized representative of the offeror (see Section 4 for template)  Official quotation, including proof of technical qualifications, (see Section 3 for scope of work, required technical qualifications, and the template to provide quotations);  A copy of the official registration and tax identification number/tax certificate (single tax/VAT);  Company’s profile or 2-3 pages introduction to company’s areas of expertise and services, and description of the team and main clients;  Narrative description of past performance for similar services (1-2 pages);  The CV of assigned designers with references from previous customers of events certifying provision of graphic design services;  10-15 examples of designs (Internet links to relevant materials) produced by the offeror. Infographics, presentations, and a multi-page guide are required.  Contact information for at least three (3) references of past or present clients indicating the relevant services carried out in the last three (3) years that best illustrate company’s qualifications and past performance. Independent verification of the references will be carried out. Chemonics reserves the right to obtain past performance information from sources other than those identified by the offeror  Evidence of Responsibility signed by authorized representative of the offeror (see Section 5).   • Inspection Checklist (check one) ☐ In order to avoid disagreements about the condition of the Premises, at the time of accepting possession of the Premises, Tenant will complete the Inspection Checklist incorporated herein by reference and attached hereto as Exhibit B and record any damage or deficiencies that exist at the commencement of the Term. Landlord will be liable for the cost of any cleaning or repair to correct damages found at the time of the inspection.   • Implementation Schedule TIME IS OF THE ESSENCE with regard to all dates and time periods set forth and/or incorporated herein. Any material modification or deviation from an approved schedule described in this Agreement shall occur only upon approval of the City and RDA, with any such approvals required to be in writing as an amendment to this Agreement, and which approvals shall not be unreasonably withheld. City shall cooperate and act promptly with respect to any and all permits or approvals necessary for completion of the Project.
