When consumers who are on parole or probation seek treatment on a voluntary basis, these self- referrals must be handled like any other self-referral to the MSHN-funded network. PROVIDER may seek to obtain consent Agreement releases to communicate with a consumer’s probation or parole agent but in no instance may this be demanded as a condition for admission or continued stay.

When individuals who are on parole or probation are referred for mandated treatment by the Michigan Department of Corrections (MDOC), the individual’s supervising parole/probation agent will send a written referral (MDOC Form CFJ-306) to the PROVIDER. The PROVIDER is required to utilize the Level of Care Determination in REMI at the time of the request for services to document screening activity and medical necessity for the appropriate Level of Care. The PROVIDER may not deny an individual an in-person assessment via phone screening. Individuals under supervision of MDOC are considered a priority population and should be offered admission within 14 days of the request for service, provided they meet medical necessity for services. Further, it is understood and agreed by PROVIDER and MSHN that; • MSHN/PROVIDER will not honor Supervising Agent requests or proscriptions for level or duration of care, services, or supports and will base admission and treatment decisions only on medical necessity criteria and professional assessment factors. • In the case of MDOC individuals, assessments should include consideration of the individual’s presenting symptoms and substance use/abuse history prior to and during incarceration and consideration of their SUD treatment while incarcerated. • All MDOC referred individuals will be provided with an assessment. If the individual does not meet medical necessity for any SUD services, the PROVIDER will give information regarding community resources such as AA/NA or other support groups. • To the extent consistent with HIPAA, the Michigan Mental Health Code and 42 CFR Part 2, and with the written consent of the individual, the PROVIDER will provide notice of an admission decision to...

  • New Consumers If New Consumers elect not to opt-out of the Program as provided in Article 3.2, such New Consumers will be automatically enrolled by Competitive Supplier in the Program. Competitive Supplier shall enroll such New Consumers in accordance with applicable Local Distributor rules.

  • Consumers Nothing in these Terms shall affect the rights of Consumers.

  • Copper Subloops Sprint will make available access to copper subloops on an unbundled basis. A copper subloop is a portion of a Copper Loop, or Hybrid Loop, and is comprised entirely of copper wire or copper cable that acts as a transmission facility between any accessible terminal in Sprint’s outside plant, including inside wire owned or controlled by Sprint, and the end-user customer premises. A copper subloop can also include intermediate devices, such as repeaters, used to establish the transmission path. Copper subloops can be used by CLEC to provide voice-grade services as well as digital subscriber line services. Access to copper subloops is subject to the collocation provisions of this Agreement. Copper subloop consists of the distribution portion of the copper loop. Sprint is not obligated to offer feeder loop plant as a stand-alone UNE.

  • NASPO ValuePoint eMarket Center a. In July 2011, NASPO ValuePoint entered into a multi-year agreement with SciQuest, Inc. whereby SciQuest will provide certain electronic catalog hosting and management services to enable eligible NASPO ValuePoint’s customers to access a central online website to view and/or shop the goods and services available from existing NASPO ValuePoint Cooperative Contracts. The central online website is referred to as the NASPO ValuePoint eMarket Center.

  • Eligible Consumers Residential, commercial, industrial, municipal, or other consumers of electricity who receive Basic Service from the Local Distributor as of the Effective Date, at one or more locations within the geographic boundaries of the Town. This includes (1) Basic Service consumers who have indicated that they do not want their contact information shared with Competitive Suppliers for marketing purposes; and (2) consumers receiving Basic Service plus an optional Green Power product that allows concurrent enrollment in either Basic Service or competitive supply. This excludes (1) Basic Service consumers who have asked their Local Distributor to not enroll them in competitive supply; (2) Basic Service consumers enrolled in a Green Power product that prohibits switching to a Competitive Supplier; and (3) consumers receiving competitive supply service.

  • Loop Provisioning Involving Integrated Digital Loop Carriers 2.6.1 Where InterGlobe has requested an Unbundled Loop and BellSouth uses IDLC systems to provide the local service to the End User and BellSouth has a suitable alternate facility available, BellSouth will make such alternative facilities available to InterGlobe. If a suitable alternative facility is not available, then to the extent it is technically feasible, BellSouth will implement one of the following alternative arrangements for InterGlobe (e.g. hairpinning):

  • Consumer If You are a consumer (defined by the FCA as any natural person acting for purposes outside his trade, business or profession), You must use reasonable care not to make a misrepresentation to insurers (which includes a failure to comply with the insurer’s request to confirm or amend particulars previously given).   • SUBLOOPS 45.1. Sprint will offer unbundled access to copper subloops and subloops for access to multiunit premises wiring. Sprint will consider all requests for access to subloops through the ICB process due to the wide variety of interconnections available and the lack of standards. A written response will be provided to CLEC covering the interconnection time intervals, prices and other information based on the ICB process as set forth in this Agreement.
