The Service will develop links with referrers to ensure streamlined pathways for patients. The Service will operate from multiple locations and depends on referrals from GPs and other clinicians. The Provider will need to evidence how it will market its services to GPs and other referrers within the NHS Eastern and Coastal Kent area. The Service is reliant on effective referral transmission on the xxx.xxx system/fax and the Choose and Book system. The Provider’s clinical staff must work collaboratively with other service providers to provide an overall streamlined service. 2.6 Relevant networks and screening programmes A number of local and national networks support the provision of a high quality service; these include:  Arthritis and Musculoskeletal Alliance (ARMA)  Musculoskeletal Collaborative Group  individual membership of nationally and professionally recognised clinical interest groups such as the: o Acupuncture Association of Chartered Physiotherapists (AACP) o Manipulation Association of Chartered Physiotherapists (MACP) o Association of Chartered Physiotherapist in Orthopaedic Medicine (ACPOM) Association of Chartered Physiotherapists in Women’s Health (ACPWH.

Interdependencies and other services. There is a dependency on the referral management centre to allocate patients referred in east Kent to each of the cataract surgery providers as deemed appropriate at the time. The Provider(s) will:  maintain appropriate information systems, which are compliant with NHS IT requirements and NHS Net.  maintain an electronic booking system which allows patient booking via the Referral Management Centre. This system is additional to, but will compliment choose and book systems  maintain systems supporting smooth and efficient booking and rescheduling of patients, including direct telephone access  ensure sufficient booking slots are made available, to meet activity requirements  contact patients to confirm their surgical and follow up appointment dates  at all stages of the pathway be prepared if requested to inform the Commissioner about the progress of patients through the system and any capacity planning that is required  following surgery, (where necessary) arrange appropriate community support, including district nursing  following surgery and before the follow-up appointment, enable patients 24 hour access via telephone to an ophthalmic nurse practitioner who if deemed appropriate, will arrange for the patient to speak with a consultant ophthalmologist during normal working hours and within one working day, if the patient suspects that all is not well with their eye that has been operated on. For the avoidance of doubt the Commissioner considers this an important part of post operative patient support  ensure that patients are appropriately followed up by the service post-operatively  ensure the patient receives optometrist refraction at 4 weeks post-operatively and this data is received, recorded and analysed regularly by the service for outcome monitoring purposes  once optometrist refraction outcome data is received, the patient will be discharged from the cataract surgery service and responsibility for the patient’s care returns to the patient’s GP and optometrist  will ensure that all equipment in use to provide this service will be of a standard acceptable to the PCT and will be compliant with guidelines for its use and maintained to manufacturers safety specifications The PCT will, through its relationship with the referral management centre:  book patients into appointment slots as per booking schedule.  confirm cataract clinic pre-operative outpatient attendance to provider.  provide the patient with the Provider’s information book...
