Client Records. 26.2.1 CONTRACTOR shall prepare and maintain accurate and complete records of clients served and dates and type of services provided under the terms of this Agreement in a form acceptable to ADMINISTRATOR.

Notwithstanding anything to the contrary, upon termination of this 18 Agreement, CONTRACTOR shall relinquish control with respect to client recordsCOUNTY data 19 to COUNTY in accordance with Subparagraph 38.2. 20 COUNTY may refuse payment for a claim if client records are 21 determined by COUNTY to be incomplete or inaccurate. In the event client records are determined 22 to be incomplete or inaccurate after payment has been made, COUNTY may treat such payment 23 as an overpayment within the provisions of this Agreement.
Client Records. 3 25.2.1 CONTRACTOR shall prepare and maintain accurate and 4 complete records of clients served and dates and type of services provided 5 under the terms of this Agreement in a form acceptable to ADMINISTRATOR.
Client Records. Original documentation shall not be removed from a hospital site by CONTRACTOR staff. The original documentation shall be placed in the locked bags provided at the site to ensure secure transportation of documents to the designated System of Care (Adult System or Care or Children’s System of Care). CONTRACTOR shall maintain adequate records of each individual client which shall include a record of services provided by the various professional and paraprofessional personnel in sufficient detail to make possible an evaluation of services, including records of client interviews and progress notes. Further, at the termination of contractual relationships between COUNTY and CONTRACTOR, COUNTY shall have such access to client records as is reasonably necessary to assure continuity of client care.

Client Records. CONTRACTOR shall maintain individual records for each client. Such records shall include identifying data, social and financial data, and a record of services provided by various personnel in such sufficient detail to make possible an evaluation by COUNTY of services rendered. COUNTY, at its sole option, may take custody and be responsible for safeguarding CONTRACTOR'S client records upon termination of this Agreement and shall thereupon act as custodian of such records for CONTRACTOR. CONTRACTOR shall be permitted access to and have a right to make copies of such records at any time. COUNTY agrees to maintain such records for such period as may be required by Title 22 of the California Code of Regulations. COUNTY agrees that such custody will conform to applicable confidentiality provisions of State and Federal law.
Client Records. Contractor shall appropriately secure all records and files to prevent access by unauthorized persons. The Contractor shall, and shall require its employees and subcontractors to comply with all appropriate federal and state laws, rules and regulations regarding confidentiality of client records.
