AMBULANCE EXPENSE INSURANCE. 8.01 Upon receipt and approval by the insurer of due proof that an eligible employee or dependent has incurred expense in respect of the use of a hospital or professional motor car ambulance or licensed air ambulance, the insurer will, subject to Article 15, reimburse the employee for such expense, provided that payment will be made only in respect of a disability where hospital confinement is required and only if the insurer is satisfied that the physical condition of the individual precludes the use of other means of transportation.

AMBULANCE EXPENSE INSURANCE. The Employer agrees to provide coverage for employees and their eligible dependents should they incur expense in respect to the use of a hospital or professional motor car ambulance. Eligible dependents shall include the spouse and dependent children from birth to twenty-one years of age and dependent children who are mentally or physically infirm to any age, of an employee. Payment will be made only in respect of a disability where hospital confinement is required and only if the insurer is satisfied that the physical condition of the individual precludes the use of other means of transportation. The coverage in and above for each employee and eligible dependents shall commence the first of the month following the termination of the probationary period for new employees. The Employer agrees to pay the full cost of the Long-term Disability Insurance Plan which, in the event of total disability, will provide employees with sixty-six percent of base earnings, beginning with the week of disability. The Plan provides that an employee will be deemed to be disabled if unable to follow his own occupation during the first two years of disability. After two years, the employee must be unable to follow any occupation for which he is suited by reason of training, education or experience. The amount of the monthly benefit payable with respect to a particular disability of an insured employee shall be based on the hourly rate in effect for the employee on the date of commencement of that disability. The benefit will cease on the earliest of the following dates: The date the employee ceases to be totally disabled;

  • Trauma Insurance All employees will be covered by an Incolink administered lump sum insurance policy providing financial compensation in the event of a major work related (ie. WorkCover) accident resulting in death or permanent total disablement. The full and precise conditions of this cover will be in accordance with the terms of the policy, but in general will provide that, in the event of a workplace accident occurring which results in either the death or total permanent disablement of a worker covered by this Agreement, a lump sum payment as specified below will made. The defined payments are: With dependants $250,000 Without dependants $150,000 This benefit has been agreed to by the company on the grounds that premium costs have been set at $7 per week/worker and will not exceed that amount.   • Fire Insurance The LESSEE shall not permit any use of the leased premises which will make voidable any insurance on the property of which the leased premises are a part, or on the contents of said property or which shall be contrary to any law or regulation from time to time established by the New England Fire Insurance Rating Association, or any similar body succeeding to its powers. The LESSEE shall on demand reimburse the LESSOR, and all other tenants, all extra insurance premiums caused by the LESSEE's use of the premises.

  • Commercial Automobile Insurance If the Grantee’s duties include the use of a commercial vehicle, the Grantee shall maintain automobile liability, bodily injury, and property damage coverage. Insuring clauses for both bodily injury and property damage shall provide coverage on an occurrence basis. The Department, its employees, and officers shall be named as an additional insured on any automobile insurance policy. The minimum limits shall be as follows: $200,000/300,000 Automobile Liability for Company-Owned Vehicles, if applicable $200,000/300,000 Hired and Non-owned Automobile Liability Coverage

  • I nsurance During the License Term, Licensee shall, at its own cost and expense, procure and continue in force such insurance policies as are required by Licensor. Such insurance shall, at a minimum include commercial general liability insurance with a combined policy limit of at least $1,000,000 or such other amount as is reasonably agreed to by the parties. Licensor shall be named as an additional named insured on all such policies of insurance. A renewal policy shall be procured not less than ten (10) days prior to the expiration of any policy. Each original policy or a certified copy thereof, or a satisfactory certificate of the insurer evidencing insurance carried with proof of payment of the premium, shall be deposited with Licensor prior to the commencement date of the term hereof and within ten (10) days of the each anniversary date thereafter. Licensee shall provide workers’ compensation and employer liability coverage as may be required by the State of Nebraska.
