Review of Personnel Files. Each faculty member shall have the right, upon request, to review the contents of his/her own personnel file. The College shall have a representative present to ensure the integrity of the file’s contents. In the event that a faculty member requests a copy of his/her personnel file more than once in a twelve (12) month period, excluding any new materials that have been added to the file since the last request, the Employer may, at its discretion, charge in accordance with Board policy. Representation: A Federation representative chosen by the faculty member may, upon the faculty member's written authorization, review a faculty member’s personnel file and/or accompany the faculty member in this review. Discipline and Performance Appraisals: Disciplinary notice, performance appraisal (faculty evaluation summary), or other similar material that is likely to have an adverse effect on a faculty member’s reputation or employment status shall be acknowledged, signed and dated by the faculty member. If an employee refuses to sign a document the College administrator will record the employee’s refusal and sign it in his or her place. Within thirty (30) calendar days after the faculty member’s signed acknowledgement, the faculty member reserves the right to include in the file a written response to such material placed in the file, and this response shall be attached to the material in question.

  • Personnel file means, for the purposes of Part III (8VAC20-90-60 et seq.), any and all memoranda, entries or other documents included in the teacher's file as maintained in the central school administration office or in any file regarding the teacher maintained within a school in which the teacher serves.

  • Title IX Personnel means any person who addresses, works on, or assists with the school district’s response to a report of sexual harassment or formal complaint, and includes persons who facilitate informal resolutions. The following are considered Title IX Personnel:

  • Personnel record means a record kept by the employer that identifies the employee, to the extent that the record is used or has been used, or may affect or be used relative to that employee's qualifications for employment, promotion, transfer, additional compensation, or disciplinary action. A personnel record shall include a record in the possession of a person, corporation, partnership, or other association who has a contractual agreement with the employer to keep or supply a personnel record as provided in this subdivision. A personnel record shall not include:

  • Contractor Personnel means Contractor’s employees and subcontractors (as well as any employees or subcontractors of those subcontractors) performing the Services.

  • Utilization review plan or "plan" means a written procedure for performing review.

  • Utilization review organization means an entity that conducts utilization review, other than a health carrier performing a review for its own health benefit plans.

  • Asset Representations Review Agreement means the Asset Representations Review Agreement, dated as of the date hereof, between the Issuer, the Sponsor, the Servicer and the Asset Representations Reviewer.

  • MERS Procedures Manual The MERS Procedures Manual, as it may be amended, supplemented or otherwise modified from time to time.

  • Project Managers means the person at Developer responsible for the project management of Product and identified in Schedule 2;

  • Clinical review criteria means the written screening procedures, decision abstracts, clinical protocols, and practice guidelines used by a health carrier to determine the necessity and appropriateness of health care services.

  • Vendor Personnel means employees, agents, independent contractors, or any other staff or personnel acting on behalf of or at the direction of Vendor or any Vendor Contractor performing or providing Services or Deliverables under this Agreement.
