Population Health Management definition

Population Health Management means the application of strategies and targeted interventions to defined populations of Members across the care continuum to improve their health and well- being. Postpartum means the twelve (12) month period of time following the end of a pregnancy. Post-Stabilization Services means Covered Services relating to an Emergency Medical Condition, provided after a Member is stabilized, to maintain the stabilized condition or, under the circumstances described in 42 C.F.R. § 438.114(e), to improve or resolve the Member’s condition. Pre-Admission Screening and Resident Review (PASRR) is governed by 42 C.F.R. § 483.100 through 483.138 for all individuals with mental illness or intellectual disability who apply to, or reside in, Medicaid certified Nursing Facilities. PASRR aims to determine if a resident is appropriately placed in the least restrictive environment and whether the individual can be appropriately served in the NF, including provision of required mental illness/intellectual disability services.

  • Virginia Stormwater Management Program or “VSMP” means a program approved by the State Board after September 13, 2011, that has been established by a locality to manage the quality and quantity of runoff resulting from land-disturbing activities and shall include such items as local ordinances, rules, permit requirements, annual standards and specifications, policies and guidelines, technical materials, and requirements for plan review, inspection, enforcement, where authorized in this article, and evaluation consistent with the requirements of this article and associated regulations.
